摘要
目的研究中老年代谢综合征(MS)患者血清睾酮(T)水平的变化,以及MS与勃起功能障碍(ED)之间的关系。方法本组共收集78例ED患者,平均年龄(55.2±10.5)岁,40例符合MS诊断标准。对患者进行血清T的测定,并利用国际勃起功能问卷(IIEF)对患者勃起功能障碍严重程度进行分级。结果 78例ED患者中,轻度患者40例,中度患者20例,重度患者18例。通过组间比较发现,MS组的ED患者年龄较非MS者高(P<0.01),腰围较非MS者长(P<0.01),高血压及糖尿病患病率较非MS组高,FT水平较非MS的低(P<0.05),IIEF-5评分较非MS的低(P<0.05);中度ED的年龄较轻度ED年龄偏大[(58.3±8.8)岁vs(53.6±9.5)岁,P<0.05],中度ED与重度ED之间的年龄差异无统计学意义(P>0.05);中、重度ED患者体内FT水平均低于轻度ED患者(P<0.05),MS患者血清FT较不合并MS者低(P<0.01),而TT在轻、中、重度患者中没有明显差异。结论中老年MS患者血清FT较不合并MS者低,且与勃起功能障碍的严重程度存在明显的相关。对于以勃起功能障碍来就诊的患者,广大医务工作者要提高警惕,全面考虑患者是否伴有代谢综合征症状和体征,对于伴有代谢综合征的ED患者,只有同时治疗代谢综合征疾病,ED的治疗才能有事半功倍的效果。
Objective To study the changes of serum testosterone （T） levels in elderly metabolic syndrome （MS） patients, and the relationship between the erectile dysfunction. Methods The total of 78 cases of ED patients, average age was （55.22.105） years, 40 cases met the MS diagnostic criteria. Serum T was determined, and used the International Index of Erectile Function Questionnaire （IIEF） grading the patients with erectile dysfunction. Results Seventy-eight cases of ED patients, including 40 patients with mild ED, 20 with moderate ED and 18 with severe ED. Through compared between the two groups it found that the age of MS group of ED patients were higher than non-MS group （P 〈 0.01）, and the waistline of MS group were longer than non-MS group （P 〈 0.01）, the prevalence of hypertension and diabetes of MS group were higher than non-MS group, the FT leve! of MS group was lower than non-MS group （P 〈 0.05）, the IIEF-5 scores of MS group was lower than the non-MS group （P 〈0.05）; the age of moderate ED patients was greater than mild ED patients [（58.3±8.8） years vs. （53.6±9.5） years, P 〈 0.05）], There was no difference in the age between mild ED and moderate ED patients, as well as severe ED patients （P 〉 0.05）; the level of FT in patients with severe ED were significantly lower than mild ED pa- tients （P 〈 0.05）,the level of serum VF in MS patients were significantly lower than non-MS patients （P 〈 0.01）, and there were no differences in TI＇ between the three. Conclusion The level of serum FT in elderly MS patients was lower than non-MS patients,and there is significant correlation between MS and erectile dysfunction. If patients seek treatment for erectile dysfunction, the majority of medical workers should be alert to MS, can not only treat with the ED syndrome.
