大剂量芬太尼麻醉对家兔不同声强下脑干听觉诱发电位及40Hz听觉稳态电位的影响

摘要 目的 :为探讨合理的麻醉深度监测指标 ,观察了大剂量芬太尼麻醉对 2 4例家兔脑干听觉诱发电位 (BAEP)及 40Hz听觉稳态电位 ( 4 0Hz ASSR)的影响。方法 :实验组分 3次静注总量 90 μg/kg芬太尼 ,对照组注射等量生理盐水。于首次给药前及给药后 5、2 5、45分钟测定 80、60、50dBnHL下BAEP及 1 2 0、1 1 0、1 0 0dBnHL下 40Hz ASSR波形变化及阈值强度。结果 :随着血浆芬太尼浓度逐渐升高 ,各声强下BAEPⅠ、Ⅱ、Ⅳ波潜伏期 ,振幅 ,Ⅰ Ⅱ、Ⅱ Ⅳ、Ⅰ Ⅳ波间期无明显改变 ,各测定点阈值强度无改变 ;而各声强下 40Hz ASSRPa、Pb波潜伏期逐渐延长 ,振幅下降 ,各测定点阈值强度逐渐增高。结论 :与BAEP相比 ,1 2 0、1 1 0、1 0 0dBnHL声强下 40Hz Objective:To explore the objective monitoring index of anesthesia depth,we studied respectively the effects of high dose fentanyl anesthsia on brain stem auditory evoked potentials(BAEP)and on 40Hz auditoy steady state responses (40Hz ASSR)in twenty four rabbits.Methods:After intravenous injection of a cumulative dose of 90μg/kg fentanyl(three equal bolus injections)in study group and equivalent volumes of saline in control group,the changes of BAEP at the sound stimulus intensity levels(SSIL) of 80,60,50dBnHL,40Hz ASSSR under SSIL of 120、110、100dBnHL and their auditory evoked potentials thresholds(AEPT)were measured respectively before the first bolus of fentanyl and at 5,25,45 min after the first bolus of fentanyl.Results:The latencies,amplitudes of wave Ⅰ,Ⅱ,Ⅳ and interwave Ⅰ Ⅱ,Ⅱ Ⅳ,Ⅰ Ⅳ of BAEP and AEPT of BAEP at three different SSIL had no change with the increasing of serum fentanyl concentrations,and the latencies of wave Pa, Pb of 40Hz ASSR at above three SSIL were progressively increasing with the increasing of serum fentanyl concentrations,at sametime, decreasing in the amplitudes and increasing in the AEPT.Conclusion:40Hz ASSR seems to be better than BAEP in monitoring the depth of anesthsia with high dose fentanyl.
