摘要
目的 :探讨Ebstein畸形中三尖瓣病理解剖特征及其临床意义。方法 :结合术前超声和术中探查研究 3 6例Ebstein畸形三尖瓣病变的特征 ,并据此选择术式。结果 :3 6例中 ,隔叶均有下移 10 0 %( 3 6例 ) ,后叶次之 94.4% ( 3 4例 ) ,前叶很少 13 .9% ( 5例 )。前叶中 97.2 %瓣体延长扩大 ,83 .3 %瓣叶质量和瓣下结构及动度基本正常。利用前叶行三尖瓣重建术 3 0例 ,均取得较好疗效。结论 :Ebstein畸形的三尖瓣病变并不仅限于下移 ,而是累及整个瓣膜装置 ,手术时应详细了解病变情况 ,在可能的情况下 ,以重建术为首选。
Objective:To elucidate the pathological anatomy of tricuspid valve in Ebstein′s malformation and discuss its clinical significance.Methods:The pathological characters of tricuspid valve of 36 patients were examined by preoperational echocardiography and surgical intervention.Results:In 36 cases,the septal leaflets displaced 100%(36 cases),followed by posterior leaflet 94.4% (34 cases) and anterior leaflets 13.9%(5 cases).The anterior leaflets were enlarged and elongated in 97.2% of the specimens and 83.3% of them had good quality and nearly normal subleaflet structure with good mobility.Satisfactory effect had received on 30 cases of tricuspid valve reconstruction by anterior leaflets.Conclusion:Ebstein′s malformation is much more than simple“downward displacement” of the leaflets,it can affect the whole leaflet device.It is necessary to make a detailed assessment of its leaflets anomaly,and reconstruction should be the first choice of surgical procedures whenever possible.
