摘要
目的 研究空腹甘油三酯水平 (TG)正常的 2型糖尿病患者餐后脂代谢的改变。方法采用改良脂肪餐负荷试验对 33例空腹TG正常 (观察组 )和 10例空腹TG增高 (高脂组 )的 2型糖尿病患者以及 10名健康对照者 (对照组 )进行餐后脂代谢研究。前者依据餐后 4小时TG(TG4h)再分为餐后正常组和餐后增高组。结果 脂餐后受试者 2~ 8小时TG水平均有所增高 ,高密度脂蛋白 胆固醇 (HDL C)都有一个轻微的低谷改变 ,其中观察组较对照组TG4h的增高和 4小时HDL C的降低差异有显著性 (P <0 .0 5 ) ;餐后总胆固醇 (TC)变化均不明显 ,组间各时段比较差异无显著性 (P >0 .0 5 )。餐后TG峰值 ,TG曲线下面积 (TG AUC)和TG峰值时间按对照组、餐后正常组、餐后增高组和空腹增高组的次序依次递增 (前两组比较和后两组比较均为P >0 .0 5 )。结论 部分空腹TG水平正常的糖尿病患者存在餐后TG水平的增高、廓清延迟和HDL C降低 。
Objective To study the change of postprandial lipid metabolism in fasting normotriglyceridemic type 2 diabetic patients. Methods Subjects were assigned to three groups: the observed group of 33 fasting normotriglyceridemic type 2 diabetic patients who were divided into two subgroups: subgroup 1 〔4 hour postprandial blood triglyceride level (TG 4h ≤2.0mmol/L)〕 and subgroup 2 (TG 4h >2.0mmol/L) according to the level of TG 4h +2s in control group; the healthy control group of 10 normal subjects and the diabetic control group of 10 fasting hypertriglyceridemic type 2 diabetic patients (hyper TG groups). Total cholesterol (TC), HDL cholesterol (HDL C), and total triglyceride (TG) were measured before and 2, 4, 6 and 8 hours after a fatty test meal (600 kcal, 45% carbohydrate, 35% fat and 20% protein). Results 2~8 hours after the test meal, a slightly postprandial increase of TG and a slightly postprandial decrease of HDL C were observed, and there was a significant differences in TG 4h and HDL C 4h between the observed group and healthy controls (P<0.05). Postprandial change in mean TC was slight and without significant differences between the two groups. Following the order of the control group, subgroup 1, subgroup 2 and the hyper TG group, the triglyceride peak value, and the area under triglyceride curve (TG AUC) over 8 hours became gradually higher and the triglyceride peak time were gradually delayed, but with no significant differences between subgroup 1 and the control group and between subgroup 2 and the hyper TG group. Conclusion The postprandial lipemic responses to an oral fat challenge in normotriglyceridemic type 2 diabetic patients including elevation of TG, delayed clearance of TG and decreased HDL C, are comparable to those of the fasting hypertriglyceridemic patients.
