重庆市某三甲医院医务人员消除艾滋病歧视调查及干预 被引量:9

Survey and Intervention on Elimination of HIV/AIDS-related Discrimination Among Medical Staff of a Third-grade Class-A Hospital in Chongqing

摘要 目的通过对综合医院医务人员的艾滋病歧视情况调查,进行消除医务人员艾滋病歧视综合宣传并评价其宣传效果。方法选择重庆市第四人民医院临床一线医务人员,采用全球基金中国艾滋病项目设计的关于艾滋病歧视态度的调查问卷,采用不记名的问卷调查方法,并对医务人员进行艾滋病知识、职业防护技能、相关伦理与道德、不歧视的相关政策的培训干预,干预前、后收回有效问卷各500份。结果医务人员已经掌握了一些艾滋病防治知识,经过培训其正确率得到提高,其正确率由干预前的93.0%,提高到干预后的97.2%,差异有统计学意义(P<0.05)。医务人员对艾滋病歧视态度客观存在,经干预,其不歧视率由干预前的49.8%,提高到65.2%,差异有统计学意义(P<0.001)。结论干预明显消除了医务人员对艾滋病歧视。但医务人员存在职业暴露风险,消除医务人员对艾滋病歧视非一日之功,需要长期持续不懈的努力。综合医院开展艾滋病防治知识培训,可通过领导支持,科教、医务和护理多个职能部门配合,分次培训,以提高培训率。 Objective To investigate the situation of AIDS-related discrimination among medical staff of a general hospital,to conduct a comprehensive propaganda and training for the elimination of HIV/AIDS-related discrimination in medical staff and then evaluate its publicity effect.Methods An anonymous questionnaire survey was conducted in clinical medical professionals who were selected from the Fourth People's Hospital of Chongqing.Questionnaires about HIV/AIDS-related discrimination designed by the Global Fund China HIV/AIDS Project were used in the survey.The training and intervention about HIV/AIDS-related knowledge,occupational protection skills,the related ethic and moral,and the non-discrimination policies were conducted in the medical professionals.Totally 1,000 valid questionnaires were retrieved before and after the intervention(each n=500).Results The medical professionals had already acquired some knowledge about HIV/AIDS,and the correct answer rate was increased to 97.2% after the intervention from 93% before the intervention,the differences were statistically significant(P〈0.05).Medical professionals' attitudes towards HIV/AIDS-related discrimination existed objectively.The non-discrimination rate was increased to 65.2% after the intervention from 49.8% before the intervention,and the differences were statistically significance(P〈0.001).Conclusions After the intervention,medical professionals' non-discriminatory attitudes towards persons with HIV/AIDS significantly improved.But there exists a risk of occupational infection in medical professionals,the goal of eliminating HIV/AIDS-related discrimination can not be reached in one step and it is necessary to make persistent efforts.To increase the training rate of HIV/AIDS-related knowledge in a general hospital could use the following methods: winning the leaders' supports,using the cooperation of science and education department,medical administration department and nursing care department,and conducting several cycles of training.
