摘要
为探讨细胞周期蛋白D1 (CyclinD1 )在肺癌发生中的作用及其与生物学行为的关系 ,采用免疫组化SP法观察CyclinD1在肺癌及癌前病变和正常肺组织的表达。结果显示 ,正常肺组织和鳞状化生的支气管上皮未见CyclinD1表达 ,在轻、中、重度不典型增生、原位癌、浸润癌 ,CyclinD1表达的阳性率逐渐升高 ,阳性细胞数也增多 ,肺癌组织中CyclinD1表达的总阳性率为 5 6 .4%。低分化癌的阳性率高于中高分化癌 (P <0 .0 5 ) ;无论鳞癌还是腺癌 ,CyclinD1阳性组的增殖活性高于阴性组 (P <0 .0 5 ) ,肺鳞癌有淋巴结转移组CyclinD1的阳性率高于无转移组 (P <0 .0 5 )。结果提示 :CyclinD1表达与肺癌的发生和增殖有关 ,而且CyclinD1表达是支气管粘膜上皮癌变的早期事件 ,说明CyclinD1可作为判断肿瘤发生可能性的标记物 ,在肺癌的早期诊断中将具有重要意义。CyclinD1表达与肺鳞癌分化不良及淋巴结转移有关 ,可能是预后不良的标志。
To explore the correlation of cyclin D1 expression with carcinogenesis and biologic behavior of lung carcinomas, the expression of cyclin D1 in human lung carcinomas, precancerous lesions and normal lung tissues was studied immunohistochemically on formalin fixed, parafin embedded sections. No cyclin D1 expression was present in normal mucosa and squamous metaplasia. With the progress of bronchial lesions from mild dysplasia to carcinoma in situ and invasive carcinoma, the positive rate and the number of immunostained cells increased. The expression levels of cyclin D1 were higher in poorly differentiated carcinomas than in well differentiated carcinomas (P<0.05). There was a correlation between cyclin D1 overexpression and proliferating activity of tumor cells (P<0.05). Squamous cell carcinomas with lymph node metastasis exhibited higher cyclin D1 expression levels compared with those without metastasis. These results suggest that the expression of cyclin D1 contributes to the pathogenesis and may be an early event in the genesis of lung carcinoma, and it is implicated in tumor differentiation, proliferation and lymph node metastasis.
