准分子激光屈光性角膜切削术治疗近视一年疗效分析 被引量:1

摘要 目的 评价准分子激光屈光性角膜切削术治疗近视及近视散光的有效性和稳定性。方法 对近视度数在-1.50~-1600D近视及近视散光患者130例211眼采用扫描式准分子激光进行治疗。患眼按术前屈光度分为3组:低中度组:-1.50~-600D,75眼;高度组:-6.25~-900D,84眼;超高度组:-9.25~-1600D,52眼。术后随访裸眼视力、矫正视力、屈光度、眼压与角膜地形图。结果 术后1年,低中度、高度、超高度近视3组裸眼视力在0.5以上者分别为97.3%、84.5%、53.8%,在1.0以上者分别为93.3%、64.3%、34.6%,屈光度在±0.50D以内者分别为88.0%、64.3%、25.0%,在±1.00D以内者分别为94.7%、81.0%、282%。角膜上皮下雾状混浊、屈光回退和激素性高眼压是主要的术后并发症,而且矫正的度数越高,并发症的发生率越高。术后角膜地形图主要为均匀圆形者占60.2%,钥匙孔或半圆形占12.3%,规则领结形占23.7%,不规则形占3.8%。结论 准分子激光屈光性角膜切削术治疗低、中、高度近视散光效果较好,对于超高度近视的治疗有效性、稳定性和安全性较差。 Objective To evaluate the efficacy and stability of excimer laser PRK for treatment of myopia and myopic astigmatism. Methods Two hundreds and eleven eyes of 130 patients ranging from -1.50 to -16 00 diopter were treated by PRK with scanning excimer laser. According to preoperative refraction, all eyes were divided into three groups. There were 75 eyes in group 1 ranging from -1.50 to -6.00 diopter, 84 eyes in group 2 ranging from -6.25 to -9.00 diopter, and 52 eyes in group 3 ranging from -9.25 to -16.00 diopter. The bestcorrected visual acuity(BCVA), uncorrected visual acuity(UCVA), refraction and corneal topography were reviewed after surgery. Results At 12 months after surgery, UCVA was better than 0.5 in 97.3%, 84.5%, 53.8% of group 1,2 and 3, respectively, and better than 1.0 in 93.3%, 64.3%, 34.6% respectively. In the above groups, 88.0%, 64.3% and 25.0% had a refraction within ±0.50 D and 94 7%, 81.0% and 28.2% were within ±1.00 D. Corticosteroid induced ocular hypertension, haze and regression were the major complications following PRK. With the ascendance of the preoperative refraction, the incidence of complications may rise. After surgery, corneal topography showed round(60.2%), semicircular(12.5%), symmetric bow tie shaped(23.7%) and irregular patterns(3.8%). Conclusions Our findings indicate that the excimer laser PRK is safe, effective and stable in the treatment of low, moderate and high myopia, but is much less effective in the treatment of extremely high myopia.
