脑脊液置换联合鞘内注药治疗结核性脑膜炎42例分析 被引量:4

Analysis of the treatment of tuberculous meningitis by cerebrospinal fluid replacement combined with intrathecal injection in 42 cases

摘要 目的探讨和研究脑脊液置换联合鞘内注药对结核性脑膜炎的治疗效果。方法选取本院近年来收治的42例结核性脑膜炎患者的临床资料,按照随机分组的原则将其分为观察组和对照组各21例,其中对照组给予抗结核和保肝、激素以及脱水剂等常规药物进行治疗;观察组在此基础上加用脑脊液置换与鞘内注药进行治疗,在治疗结束后统计两组患者的治疗情况和相关指标并进行对比。结果观察组的总有效率和显效率分别为95.24%和71.43%,对照组的总有效率和显效率分别为71.43%和52.38%,两组对比差异具有统计学意义(P<0.05)。同时观察组患者的临床症状消失时间、压力、脑脊液细胞和脑脊液蛋白的恢复时间等均明显优于对照组,差异具有统计学意义(P<0.05)。结论脑脊液置换联合鞘内注药对结核性脑膜炎的治疗有着令人满意的效果,是安全可靠的,值得在临床上加以推广和应用。 Objective To explore and study the treatment of tuberculous meningitis by cerebrospinal fluid replacement combined with intratheeal injection. Methods Removal 42 cases of clinical data of patients with tuberculous meningitis treated in our hospital treated recent years, accordance to the principle of randomization divided into observation group and control group, 21 cases in each group. The control group was given anti-TB and liver, hormone and dehydration agents' conventional drug treatment: On this basis, observation group added the method of cerebrospinal fluid replacement com- bined with intratheeal injection. After treatment, treatment status and related indicators of the two groups of patients were recorded and compared with each othe. Results In observation group, the total effective rate and markedly effective rate were 95.24% and 71.43%, the total effective rate and effective rate of the control group were 71.43% and 52.38%, the dif- ference was statistical significance between the two groups (P 〈 0.05). While clinical symptoms disappeared time, pressure, CSF cells and CSF protein recovery time of observation group patient were significantly better than those of the control group, the difference was statistical significance (P 〈 0.05). Conclusion Treatment of tuberculous meningitis by cere- brospinal fluid replacement combined with intratheeal injection get satisfactory results, is safe and reliable, it is worth to be promoted and applied in the clinical.
