摘要
目的 探讨 CD15抗原的表达及含量与胆囊癌的临床病理意义。方法 应用微波 - L SAB免疫组织化学法和图像分析技术 ,检测 45例胆囊癌、17例胆囊腺瘤和 10例慢性胆囊炎组织中 CD15抗原表达及含量。结果 CD15抗原表达阳性率及阳性细胞积分光密度值 ,在胆囊癌中显著高于胆囊腺瘤和慢性胆囊炎 (P<0 .0 5 ,P<0 .0 1) ;胆囊癌 、 级显著高于 级 (P<0 .0 5 ,P<0 .0 1) ;有转移的肿瘤显著高于无转移的肿瘤 (P<0 .0 2 5 ,P<0 .0 1)。结论 CD15抗原表达与胆囊癌的发生、细胞增殖、分化和转移相关。对判断胆囊癌的恶性程度、预测生物学行为和预后具有一定临床意义。
Objective To study the clinicopathological significance of CD15 antigen expression.Methods CD15 expression and its contents were evaluated by microwave-LSAB immunohistochemical technique and image analysis in 45 cases with gaubladder carcinoma,17 adenoma with the gallbladder and 10 cases with chronic cholecystitis. Results Positive rate of CD15 and its integral optic density in the carcinoma of gallbladder were 71.1%(32/45)and 3.96±3.24 respectively,significantly higher than 35.3%(6/17) and 0.94±0.32 in the adenoma of gallbladder,20%(2/10) and 0.37±0.12 in chronic cholecystitis(P<0.05,P<0.01).In grade Ⅱ～Ⅲ,they were 89.5%(17/19) and 5.39±1.67,significantly higher than 57.7%(15/26) and 2.18±3.12 in grade Ⅰ (P<0.05,P<0.01).In cases with positive lymph node cases,they were 87.5%(21/24) and 6.47±3.52,significantly higher than52.4%(11/21) and 3.31±2.13 in negative ones(P<0.025,P<0.01).Conclusion CD15 expression could be involved in the aggresiveness of gallbladder cancer and might be a useful indicator to evaluate the malignancy and biological features,and could be considered as a good prognostic predictor for patients with the carcinoma of gallbladder.
