摘要
目的探讨胰腺癌患者外周血CD8+T淋巴细胞亚群及CD8+T淋巴细胞上CD28分子和CD103分子的表达及其临床意义。方法应用流式细胞术对52例胰腺癌患者(胰腺癌组)和40名健康体格检查者(对照组)的外周血淋巴细胞中CD8及CD28分子表达情况进行检测;再选取10例胰腺癌患者,分别检测其外周血、癌组织及癌旁组织中CD103分子的表达。结果胰腺癌组CD8+CD28+T淋巴细胞比例为(9.88±4.32)%,较对照组的(14.12±4.95)%显著降低(P<0.01);CD8+CD28-T淋巴细胞比例为(13.13±5.52)%,与对照组[(11.41±4.01)%]的差异无统计学意义(P>0.05)。淋巴结转移组CD8+CD28+、CD8+CD28-T淋巴细胞比例分别为(10.92±4.54)%、(13.62±4.52)%,无淋巴结转移组分别为(10.54±3.83)%、(14.15±3.27)%,两组间的差异均无统计学意义(P值均>0.05)。Ⅲ期胰腺癌患者CD8+CD28+T淋巴细胞比例为(6.45±1.22)%,显著低于Ⅰ、Ⅱ、Ⅳ期的(13.01±2.31)%、(10.79±1.64)%、(10.5±1.45)%(P值分别<0.01或0.05)。胰腺癌患者癌组织中CD8+T淋巴细胞上CD103分子表达率为26%,显著高于外周血及癌旁组织中的2%及8%(P值均<0.01)。结论胰腺癌患者外周血中CD8+CD28+T淋巴细胞的比例下降和功能减退,E-cadherin/αEβ7信号通路异常,可能与胰腺癌患者机体免疫力下降有关。
Objective To explore the expression of CD28 and CD103 on CD8+ T lymphocytes in the peripheral blood of pancreatic cancer patients and its clinical significance. Methods Flow cytometry was used to determine the expression of CD8 and 0D28 on T lymphocytes in the peripheral blood of 52 patients with pancreatic cancer and 40 healthy volunteers. The expression of CD103 in the peripheral blood, cancer tissue and adjacent tissue of cancer was also assessed in 10 patients with pancreatic cancer. Results The level of CD8+ CD28+ expression in the pancreatic cancer patients was significantly lower than that in healthy volunteers (I-9. 88-I- 4.32] % vs. 1-14.12±4.95] %, P〈 0.01 ). There was no significant difference in the level of CD8+ CD28- between the pancreatic cancer patients and healthy volunteers ([ 13. 13±5.52] % vs. [11.41±4.01] %, P 〉 0.05). The levels of CD8+ 0D28+ and CD8+ CD28- expression on peripheral blood lymphocytes in the patients with lymph node metastasis were ( 10.92±4.54) % and ( 13.62±4.52) %. And the levels in the patients without lymph node metastasis were ( 10.54±3.83) % and ( 14. 15±3.27) %. There were no significant differences in CD8+ CD28+ and CD8+ CD28- expression between the two groups (P〉0.05). The level of CD8+ 0D28+ in stage Ⅲcancer patients ([6.45±1.22] % ) was significantly lower than that in stage I ([ 13.01±2.31 ] % ), stage Ⅱ (1-10.79±1.641%) and stage ([10.5±1.451%, P〈0.05 or 0.01). The expression of 0D103 on tumor- infiltration lymphocytes (26%) was significantly higher than that on peripheral blood lymphocytes (2%) and lymphocytes of cancer-adjacent tissues (8%, P〈0.01). Conclusion The decrease and hypofunction of CD8+CD28+ cell and insufficiency of E-cadherin/αEβ7 interactions may be related to the impaired immunity of pancreatic cancer patients.
