摘要
目的探讨全结肠灌洗术后I期切除吻合治疗左半结肠癌并梗阻的方法及可行性。方法对我院于2005年4月至2011年5月行全结肠灌洗术后I期切除吻合治疗25例左半结肠癌并梗阻的临床资料进行回顾性分析。结果术后并发切口感染2例,肺部感染2例,无1例发生吻合口漏或腹腔感染,全组无手术死亡,10～15d临床治愈出院。结论全结肠灌洗术后I期切除吻合治疗左半结肠癌并梗阻可使患者免去分期手术、多次手术的痛苦;只要进行充分的围手术期处理,全结肠灌洗术后I期切除吻合治疗左半结肠癌并梗阻安全可行。
Objective To explore the feasibility and method of colonic irrigation for malignant left colonic obstruction. Methods Patients with malignant bowel obstruction in left colon treated by full colonic irrigation in Guangji hospital from 2005 to 2011 were analyzed retrospectively. Results 25 patients, including 4 cases of splenic colon, 6 cases of descend colon, 13 cases of sigmoid colon and 2 cases of the junction of rectum and sigmoid colon were underwent full colonic irrigation before anastomosis. No serious complica- tion, such as deaths, anastomotic leakage or intra-abdominal infections were found after operation, post- operative wound infection in 2 case, pulmonary infection in 2 cases, patients stayed in hospital about 10 to 15 day after operation. Conclusion It is feasibiity and safety for resection and anastomosis in patients with malignant left colonic obstruction after a full colonic irrigation.
