摘要 目的:探讨转录因子GATA-4在卵巢性索间质肿瘤中的表达及意义。方法:采用免疫组织化学方法分别检测20例正常卵巢、45例卵泡膜细胞瘤、28例颗粒细胞瘤及7例支持-间质细胞瘤中GATA-4的表达情况,分析其临床病理意义。结果:GATA-4在正常卵巢、卵泡膜细胞瘤、颗粒细胞瘤及支持-间质细胞瘤中的高表达率分别为100%、89%、68%、71%。颗粒细胞瘤组中GATA-4的表达低于卵泡膜细胞瘤组及正常卵巢组,差异有统计学意义(P<0.05),而卵泡膜细胞瘤组与正常卵巢组间的表达差异无统计学意义(P>0.05),支持-间质细胞瘤组与正常卵巢组、卵泡膜细胞瘤组、颗粒细胞瘤组的表达差异均无统计学意义(P>0.05)。在颗粒细胞瘤中,不同临床分期GATA-4的表达差异有统计学意义(P<0.05);而不同的组织学类型GATA-4的表达差异无统计学意义(P>0.05)。结论:GATA-4的高表达是正常卵巢性索间质细胞分化成熟的一个标记,且在卵巢性索间质肿瘤的分化表型中起作用。GATA-4在卵巢颗粒细胞瘤中的表达降低可能代表了肿瘤细胞的分化障碍。检测GATA-4的表达可能有助于判断卵巢性索间质肿瘤的生物学行为,指导卵巢颗粒细胞瘤的治疗及预测预后。 Objective: To investigate the expression of GATA-4 transcription factor inovarian sex cord-stromal tumors and explore its significance. Methods: The expression of GATA-4 was detected in 20 cases of normal ovaries, 45 ovarian theca cell tumors, 28 ovarian granulosa cell tumors and ? ovarian Sertoli-Leydig cell tumors by immunohistochemical staining. The clinicopathological significance was analyzed. Results: The high expression rates of GATA-4 in normal ovaries, ovarian theca cell tumors, ovarian granulosa cell tumors and Sertoli-Leydig cell tumors were 100%, 89%, 68% and 71%, respectively. The GATA-4 expres-sion of ovarian granulosa cell tumors was significantly lower than that of ovarian theca cell tumors and normal ovaries, respectively (P〈0.05), while there was no significantly different expression between ovarian theca cell tumors and normal ovaries (P〉0.05). The GATA-4 expres- sion of Sertoli-Leydig cell tumors was not significantly different compared with the other three types of specimens (P〉0.05). In ovarian granulosa cell tumors, there were significant differences among different clinical stages (P〈0.05), but there were no significant differ- ences between different histological types (P〉0.05). Conclusion: The high expression of GATA-4 is a marker of maturely differentiated normal ovarian sex cord-stromal cells, and it plays a role in the differentiation of ovarian sex cord-stromal tumors. The lower expression of GATA-4 in ovarian granulosa cell tumors may represents the arrested differentiation of tumor cells. To detect the expression of GATA-4 may be helpful to determine the biological behavior of ovarian sex cord-stromal tumors, and guide the treatment, predict the prognosis of ovarian granulosa cell tumors.
