摘要
目的探讨局部激素及抗病毒药物治疗角膜内皮炎的疗效。方法选取本院眼科2009年1月～2011年6月收治的角膜内皮炎患者30例,包括盘状型、弥漫型和线状型,随机分为局部药物治疗组和局部+全身联合药物治疗组,局部药物治疗组应用更昔洛韦眼用凝胶和妥布霉素地塞米松滴眼液治疗,局部+全身联合药物组除上述局部用药外,联合口服阿昔洛韦和泼尼松。观察两个治疗组中各型角膜内皮炎患者视力恢复时间。结果两种用药方式组中的盘状型及弥漫型角膜内皮炎患者恢复的视力时间相比差异无统计学意义(P>0.05),采用局部用药的线状角膜内皮炎患者视力恢复时间要明显长于联合用药的患者(P<0.001)。结论各型角膜内皮炎患者均可通过局部药物治疗得到较好的治疗效果,局部用药同时联合全身用药能够缩短线状型角膜内皮炎患者病程。
Objective To investigate the efficacy of topical antiviral agents and corticosteroids in the treatment of corneal endotheliitis. Methods Thirty cases with corneal endotheliitis, including disciform, diffuse and linear endotheliitis from Jan. 2009 to Jun. 2011 in Xiangxiang People＇s Hospitial were divided randomly into two groups according to the difference treatment： topical treatment group and combination treatmentgroup （topical treatment with a systemic combination）. Topical treatment group was administered with ganciclovir ophthalmic gel, tobramycin and dexamethasone eye ointment, while com- bination treatment group was administered with both local and systemic medicines （acyclovir and prednisone）. The recovery time of the vision of patients in two groups was observed. Results The time of vision recovery in patient with disciform and diffuse endotheliitis was no marked difference in a local group and the combined application group （P 〉 0.05）. The time of vision recovery in patient with linear endotheliitis in a local group was significantly higher than that in the combined appli- cation group （P 〈 0.001）. Conclusion The topically use of antiviral agents and corticosteroids is effective in all kinds of endotheliitis. However, patients with linear endotheliitis need to be administered systemically with antiviral agents and cor- ticosteroids as well to quicken the recovery.
