摘要
目的调查婴幼儿变态反应性疾病发病的危险因素,指导临床采取有效的早期预防和干预措施。方法随机采集181份新生儿脐血标本,应用ELISA方法测定脐血IgE水平,并通过问卷调查方式进行为期3年的跟踪随访,观察各种变态反应性疾病的发生情况。采用SPSS统计软件进行单因素分析和多因素Logistic回归分析,明确各种因素对变态反应性疾病发生的影响。结果本研究共调查15种危险因素,单因素分析表明,患变态反应性疾病组脐血IgE水平、家族过敏史阳性率、人工喂养率、肥胖病发生率、母妊娠高血压综合征及早孕反应发生率均明显高于未患变态反应性疾病组,差异有统计学意义(P<0.05);将以上述6种因素带入多因素Logistic回归分析,结果提示阳性家族过敏史、脐血IgE升高、人工喂养方式、肥胖是影响婴幼儿变态反应性疾病的主要危险因素(P<0.05)。结论阳性家族过敏史、脐血IgE水平升高、人工喂养方式、肥胖是影响婴幼儿变态反应性疾病的主要危险因素,母妊娠高血压综合征或早孕反应会增加婴幼儿变态反应性疾病的风险。
Objective To investigate the risk factors of infant allergic diseases,in order to guide clinical to adopt early prevention and intervention measures effectively.Methods 181 neonatal umbilical cord blood specimen were collected and the levels of cord blood IgE were measured by ELISA.Questionnaires were carried out to proceed three-years follow-up,the occurrence condition of allergic diseases was observed.The influence of various factors on allergic diseases was disclosed by univariate analysis and multi-factors Logistic regression analysis with SPSS statistical software.Results 15 risk factors were investigated in the study.Univariate analysis showed that the levels of cord blood IgE,positive rate of family allergy history,artificial-feeding rate,the incidence of adiposis,incidence of pregnant hypertension syndrome and morning sickness of pregnancy of allergic diseases group were significantly higher than those of the group without allergic diseases（P 0.05）.Multivariate Logistic analysis showed that family allergy history,high level of cord blood IgE,artificial feeding,obesity were the major risk factors in infants with allergic diseases（P 0.05）.Conclusion The family allergy history,high level of cord blood IgE,artificial feeding,obesity were the major risk factors in infants with allergic diseases.Pregnant hypertension syndrome or morning sickness of pregnancy will increase the risk of allergic diseases.
