摘要
目的分析冰敷对Schatzkeer V型胫骨平台骨折双切口术后隐性失血及肢体肿胀的影响。方法对2009年3月～2011年4月在我科行Schatzkeer V型胫骨平台骨折双切口手术的49例患者进行回顾性分析,按照术后是否进行切口周围冰敷分为冰敷组(26例)和非冰敷组(23例)。通过围手术期血红蛋白变化来推算患者隐性失血量(Hbl),再测量出患者术后第1～3 d的术侧胫骨平台平面肢体周径较术前相比增加的平均值,对两组进行统计学比较分析。结果在冰敷组,隐性失血量平均为(26.17±8.08)g/L,肢体周径增加平均值为(1.81±0.50)cm;在非冰敷组,隐性失血量平均为(33.04±10.66)g/L,肢体周径增加平均值为(2.53±0.61)cm,两组隐性失血量及肢体周径变化平均值的差异均有统计学意义(P<0.05)。结论在复杂的胫骨平台骨折,尤其是Schatzkeer V型骨折,采用内外侧双切口治疗后,冰敷可减少隐性失血量及肢体肿胀,从而有益于患者切口的愈合。
Objective To analyze the effects of ice compress on hidden blood loss and swelling of limb after the double incision for Schatzkeer V type of tibial plateau fracture. Methods A retrospective analysis was made among 49 patients with Schatzkeer V type of tibial plateau fracture receiving double incision in our department from March 2009 to April 2011. According to whether the patients received ice compress around the incisal opening after the operation, they were divided into ice compress group（ 26 cases）and non- ice - compress group （23 cases ）. Their hidden blood loss （HBL）was calculated according to the changes in perioperative hemoglobin. Detection was made in their increasing average value of planar limb circumference in d 1 - 3 after the operation. Statistical comparison was made between the two groups. Results In the ice compress group,the average HBL was（26.17±8.08）g/L, and the increasing average value of planar limb circumference was（ 1.81 ±0.50）cm. In the non - ice - compress group,the average HBL was （33.04 ± 10.66） g/L, and the increasing average value of planar limb circumference was （ 2.53± 0.61 ） cm. There were significant differences in the HBL and the changing average value of planar limb circumference between the two groups （P 〈 0.05 ）. Conclusion For complicated fracture of tibial plateau especially the Schatzkeer V type of fracture, the ice compress after the medial and lateral double incision can reduce the HBL and limb swelling and is beneficial to the healing of patients＇incisal opening.
