摘要
目的了解产β-内酰胺酶大肠埃希菌临床检出率、分布和耐药性,为临床治疗产β-内酰胺酶菌感染提供依据。方法收集临床分离的202株大肠埃希菌,统计菌株来源、分布。菌株分离及培养按常规方法进行;细菌鉴定采用VITET-2Compact全自动微生物分析仪;采用试纸法检测β-内酰胺酶;采用微量肉汤稀释法测定产β-内酰胺酶大肠埃希菌对10种抗生素的MIC值,分析其耐药性。结果 202株大肠埃希菌中检出167株产β-内酰胺酶,检出率为82.67%。产β-内酰胺酶大肠埃希菌的标本来源以尿液为主,其次为口痰、消化道分泌物和伤口分泌物等。科室以普外科、肾内科、泌尿外科和和呼吸内科为主。产β-内酰胺酶菌株对多种抗生素均表现出较强耐药性,其中对氨苄西林的耐药率为100%,对亚胺培南不耐药。结论临床分离大肠埃希菌产β-内酰胺酶检出率高,耐药率较高。治疗产β-内酰胺酶菌感染应根据体外药敏试验结果选用敏感抗生素。
Objective To evaluate the distribution,rate of clinical detection,and antimicrobial resistance of β-lactamase-producing Escherichia coli to provide a basis for clinical treatment for β-lactamase-producing bacteria.Methods Two hundred and two clinical isolates of E.coli were collected from this hospital and the sources of strains and their clinical distribution were statistically analyzed.The bacteria were isolated and cultured using conventional methods.Bacteria were identified by automatic bioanalysis with the VITEK-2 Compact analyzer.A test strip was used to determine whether a strain produced β-lactamases or not.Antimicrobial susceptibility test: Micro-broth dilution was carried out to determine the MIC of 10 antibiotics in response to producing β-lactamases and the resistance of that E.coli was analyzed.Results Of the 202 clinical isolates of E.coli,there were 167 strains(82.67%) of β-lactamase-producing E.coli.The main source of β-lactamase-producing E.coli was urine,followed by sputum,gastrointestinal secretions,and wound secretions.Strains were mostly from General Surgery,Nephrology,Urology,and Respiratory Medicine.β-lactamase-producing E.coli had a high rate of antimicrobial resistance.Conclusion β-lactamase-producing E.coli was detected at a high rate in clinical isolates,and this β-lactamase-producing E.coli was relatively resistant to antimicrobials.Drugs to treat infection caused by β-lactamase-producing strains should be chosen depending on the results of antimicrobial susceptibility testing to avoid the prevalence of β-lactamase-producing strains.
