摘要
目的在亚低温治疗重型颅脑损伤患者过程中监测颅内压、血氧饱和度、血气分析及电解质的变化,来探讨亚低温的脑保护机制。方法将40例重型颅型损伤患者随机分为亚低温组和常温组,采用常规腰穿动态监测脑脊液的压力,指套型数字显示型无创伤监护仪进行脉搏血氧饱和度的连续监测,动态监测动脉血气及静脉血电解质的变化。结果亚低温可以降低颅内压,提高患者动脉血氧分压,降低脉搏血氧饱和度,导致低血钾。结论亚低温可以改善脑缺氧,同时改善全身其他器官的能量代谢,纠正酸中毒,对重度颅脑损伤后的继发性损伤起到一定的保护作用。
[Objective] To monitor intracranial pressure,blood oxygen saturation,blood gas analysis and electrolyte changes,and explore the hypothermia brain protection mechanism on patients with severe head injury receiving therapeutic hypothermia.[Methods] A total of 40 cases of severe cranial injury patients were randomly divided into mild hypothermia group and normal group,using conventional lumbar dynamic monitoring of cerebrospinal fluid pressure,finger type digital display type non-invasive monitor for pulse oxygen saturation monitoring,ambulatory monitoring of arterial blood gas and electrolyte changes in venous blood.[Results] hypothermia could reduce intracranial pressure,improve patients' arterial partial pressure of oxygen,reduce pulse oxygen saturation,and lead to hypokalemic.[Conclusion] Mild hypothermia can improve cerebral oxygen supply,improve the energy metabolism of other body organs,correct acidosis,and has a protective function against severe traumatic brain injury after the secondary injury.
