摘要
目的探讨冷冻器辅助摘除眶内肿瘤的手术方法、手术技巧,提高一次性肿瘤完整摘除率,减少肿瘤的复发率。方法在最易显露肿瘤的方位,用最佳的手术入路,逐步分离暴露肿瘤,使分离暴露的肿瘤表面大于冷冻头面积2～3 mm,将冷冻头置于已分离的肿瘤区,同时开启冷冻,调整温度值至-80℃左右,在冷冻头冻住肿瘤不易脱落的状态下,边轻轻往外牵拉肿瘤,边用骨膜剥离器仔细分离肿瘤表面的正常组织,直至肿瘤完整摘出。结果使用冷冻器辅助方法摘除眼部肿瘤20例,其中一次性完整摘除17例(海绵状血管瘤8例、黏液囊肿2例泪腺多形性腺瘤3例、神经胶质瘤2例、脑膜瘤1例、皮样瘤1例),3例边界不清的肿瘤(睑板腺癌1例、肿块型炎性假溜2例)。因与周围组织粘连少许残留。结论使用冷冻器辅助方法摘除眼部肿瘤,不需特殊的设备,对囊性肿瘤和边界清晰的实体性肿瘤一次性摘除率高,对边界不清晰的肿瘤摘除也有帮助,可以避免用组织钳夹破肿瘤,使肿瘤难以完整摘除,并可能引起的肿瘤复发。
【Objective】 To investigate the freezing method of surgical removal of orbital tumors,surgical techniques and improve the rate of complete removal of tumor and reduce the rate of tumor recurrence.【Methods】 In the most easy to reveal the position of the tumor,with the best surgical approach,the gradual separation of the exposed tumor,so that the separation of the exposed surface of the tumor than the frozen head area of 2～3 mm,the frozen head in separate tumor areas,while open the freezer,adjust the temperature to-80℃,the head frozen in the frozen tumor easy to fall off the edge gently out of the stretch tumor,while carefully separated from the surface of the normal tissue of the tumor with periosteal stripping,Until the tumor complete extraction.【Results】 The use of frozen removal of ocular tumors 20 cases,including one-time complete removal of the 17 cases（8 cases of cavernous hemangioma,2 cases of mucocele,3 cases of the lacrimal gland pleomorphic adenoma,2 cases of glioma,1 cases of meningioma,1 cases of dermoid）,3 cases of ill-defined tumors（1 cases of meibomian gland and 2 cases of mass in inflammatory pseudotumor of the slip to removal of ocular tumors and surrounding tissue adhesions little residue.【Conclusion】 Freezing method for removal of ocular tumors do not need special equipment,one-time removal rate,and also help clear boundary of tumor removal,can be avoided with tissue forceps to break the tumor,the tumor is difficult to complete removal of the cystic tumor and clear boundary entities tumor,and may cause tumor recurrence.
