摘要
目的探讨肺隐球菌病的临床表现,诊断及治疗情况,以提高对该病的认识。方法收集该院2005年1月～2010年6月确诊的14例肺隐球菌病例资料,与经筛选后的1999～2009年中国学术期刊网及PUBMED数据库收录的421例肺隐球菌病的资料共同荟萃分析,以分析其临床特点。结果肺隐球菌病超过一半发生于无基础疾病及易感因素者,症状、影像学表现形式多样,易并发隐球菌性脑膜炎;隐球菌抗原乳胶凝结试验、CT引导下经皮肺活检及经支气管镜肺活检对诊断有重要意义,目前临床应用较少;治疗手段包括手术、抗真菌药物及联合治疗。结论目前临床对于该病的认识不足,常误诊为肿瘤、结核及一般感染,应强调上述3种检查方法的引用。单纯手术治疗治愈率较低,且易产生并发症。
【Objective】To investigate the clinical manifestation,diagnosis and managemnet of pulmonary cryptococcosis.【Methods】Report the clinical features of 14 cases diagnosed and treated in our hospital from 2005/1 to 2010/6,a retrospective analysis of these 14cases and 421 cases retrieved from CNKI and Pubmed database.【Results】More than half of the cases are without related diseases and risk factors,syptoms and imaging manifestations vary,complication cryptococcal meningitis is of high incidence.Detection of cryptococcal antigens by latex agglutination test,CT-guided percutancous cutting needle biopsy and transbronchoscopic lung biopsy are of diagnostic importance.Management involves resection with operation and application of anti-fungi drug as well as combined therapy.【Conclusion】Cognition to the disease is insufficient,causing misdiagnoses.Those three diagnostic methods mentioned above should be emphasized in clinical practice.Surgery should be accompanied with anti-fungi drugs to avoid low cure rate and complications.
