摘要
目的探讨慢性乙型肝炎(CHB)患者肝细胞脂肪变与肝脏HBsAg和HBcAg表达的关系。方法选取2005年1月至2008年6月间天津市传染病医院经肝活组织检查确诊的CHB合并肝脂肪变患者147例,另选取与其血清HBVDNA接近的无肝脂肪变的CHB患者149例,比较2组肝脏HBsAg、HBcAg免疫组织化学染色及肝脏损伤的差异。统计分析采用t检验和)X^2检验。结果与无肝脂肪变组相比,肝脂肪变组平均年龄和体质指数较高(t值分别=-3.31和-6.57,P值均〈0.01),肝组织中重度炎性反应、明显纤维化及HBsAg强阳性染色的比例则较低(分别为30.6%比15.4%、26.5%比12.8%、23.1%比6.7%,X^2值分别=9.63、8.92、15.76,P值均〈0.01),HBcAg强阳性染色比例亦呈下降趋势。与肝脂肪变程度F1至F2级者比较,F3至F4级者肝脏HBsAg、HBcAg强阳性染色比例呈下降趋势。结论肝脂肪变影响CHB患者肝组织内HBsAg、HBcAg的表达,随肝脂肪变的出现及加重,HBsAg、HBcAg表达及肝组织损伤程度均呈下降趋势。
Objective To explore the association between hepatic steatosis and the liver tissue expression of HBsAg and HBcAg in chronic hepatitis B (CHB) patients. Methods From January 2005 to June 2008, a total of 147 CHB patients with hepatic steatosis diagnosed by liver biopsy and other 149 CHB patients without hepatic steatosis but with similar HBV DNA titer were enrolled. The differences of HBsAg and HBcAg immunostaining and liver injury in these two groups were compared. The data were analysed using t test and chi square test. Results Compared with non-steatosis group, the average age and body weight index of hepatic steatosis group were higher (t values were -3. 31 and -6. 57, both P〈0.01). The percentage of moderate to severe hepatic inflammation in liver, obvious hepatic fibrosis and the strong positive HBsAg staining was lower (30.6 % vs 15.4 %;26.5 vs 12.8% ; 23.1% vs 6. 7% ; X^2 =9.63, 8.92, 15.76; all P〈0.01), and the percentage of strong positive HBcAg staining was also in downtrend. Compared with degree F1 and F2 of liver steatosis, the percentage of HBsAg and HBcAg strong positive staining in liver tissues of degree F3 and F4 was in downtrend. Conclusions Hepatic steatosis affected the expression of HBsAg and HBcAg in liver tissue of CHB patients. As hepatic steatosis appeared and became more severe, both expression ofHBsAg and HBcAg and the degree of liver injury were in downtrend.
