摘要
目的对氯沙坦和氨氯地平在原发性高血压(PH)患者的临床治疗作用进行综合比较。方法将128例PH伴左心室肥厚(LVH)的患者随机分为氯沙坦组(L,n=65)和氨氯地平组(A,n=63)。L组晨服氯沙坦50mg/d,A组晨服氨氯地平5mg/d,观察12周。两组分别于治疗前后测定血压、血尿酸和超声心动图测定左心室舒张末内径(LVDd)、舒张期室间隔厚度(LVSd)、左心室后壁厚度(LVPWd)、左心室重量指数(LVMI)的变化。结果用药12周后,两组血压均较用药前显著下降(P<0.01),组间比较无差异(P>0.05);两组血尿酸均下降,与用药前比较,氯沙坦组下降差异有显著显著性(P<0.01),氨氯地平组下降无显著性(P>0.05)。氯沙坦组LVDd、LVSd、LVPWd、LVMI明显下降(P<0.01),而在氨氯地平组则变化不明显(P>0.05)。结论氯沙坦与氨氯地平抗高血压疗效相似,但氯沙坦除降压外还有明显的降尿酸和减轻左心室肥厚的临床治疗作用。
Objective To compare the effect of losartan and amlodipine in primary hypertension patients on blood pressure,uric acid and left ventricular hypertrophy.Methods The 128 patients with hypertension combining left ventricular hypertrophy were randomly divided into group L（losartan 50mg qd,n=65） and group A（amlodipine 5mg qd,n=63）.Changes of blood pressure and serum uric acid was observed after 12 weeks treatment.Left ventricle diastolic dimension（LVDd）,interventricular spetum thickness（LVST）,left ventricle mass index（LVMI）,left ventricle posterior wall thickness（LWPVT） were measured by ultrasound cardiography before and after 12 week＇s treatment respectively.Results The blood pressure significantly decreased on week 12 in group L and group A（P0.01）,and there was no significantly difference between group L and group A（P0.05）.While hyperuricemia significantly decreased in losartan group（P0.05）;Serum uricacid scarcely changed in amlodipine group（P0.05）.The LVDd,LVSd,LVPWd and LVMI of group L decreased significantly（P0.01） but rarely changed in group A（P0.05）.Conclusion Losartan has the same effect on blood pressure with amlodipine,besides,losartan has the clinical effect on decreasing serum uric acid and left ventricular hypertrophy.
