髋关节置换术治疗老年偏瘫侧股骨颈骨折 被引量：9

摘要 [目的]探讨老年脑血管意外偏瘫侧股骨颈骨折的治疗方法。[方法]采用人工髋关节置换术治疗老年脑血管意外偏瘫侧股骨颈骨折GardenⅢ、Ⅳ型25例。患者均有不同程度内科疾病,积极治疗内科疾病和充分术前准备后,19例采用人工股骨头置换,6例采用全髋关节置换,术后保持患肢外展中立位,继续积极治疗内科疾病,预防感染和并发症。[结果]出现人工股骨头脱位1例,下肢深静脉栓塞2例,泌尿系感染2例,均治愈。远期出现假体松动下沉3例,需拐扶行走。其余恢复伤前行走能力。[结论]老年脑血管意外偏瘫侧股骨颈骨折患者,由于伴有不同程度内科疾病,长期卧床更容易出现并发症,应积极手术治疗。术前要积极治疗内科疾病,完善相关术前准备,术中、术后要注重并发症的预防,加强基础护理及后期的康复训练。
