摘要 目的研究高血糖对中风预后的影响,探讨中风后高血糖的时限性和最佳血糖浓度。方法研究中风患者神经功能缺损程度及病死率与血糖浓度的关系;平均血糖浓度与总体疗效的关系,探讨中风后血糖变化的规律和最佳血糖浓度。结果高血糖组病死率明显高于非高血糖组。死亡组血糖水度明显高于存活组。中风后神经功能缺损程度与血糖浓度呈正相关,总体疗效与平均血糖浓度呈负相关。脑出血、脑梗死最佳血糖浓度分别为4~5mmol/L和3~4mmol/L。中风后高血糖持续10天左右。结论高血糖使中风患者神经功能缺损加重、病死率提高、预后变差。最佳血糖浓度3~5mmol/L。适当限制中风患者葡萄糖的摄入,给予一定量的胰岛素治疗,以预防、控制高血糖,可能有助于改善预后。 Objective To study the effect of hyperglycemia (HG) to the prognosis of stroke and to explore the best blood glucose concentration (BGC) as well as the time specificity of HG after stroke. Metkods The study was focused on the relation between the degree of neural function defect of stroke patients and the mortality rate with BGC,and the relation between the mean blood glucose concentration (MBGC) and the total treatment effect. The best BGC range of stroke and the regulation of HG after stroke were also included in the study. Results The mortality rate of HG groupwas significantly higher than that of non-HG group. The highest BGC of the dead group was significantly higher than that of the survival group. The degree of the neural function defect of stroke patients was positively related to the BGC. The total treatment effect was negatively related to the MBGC. The best BGC was 4-5mmol/L in cerebral hemorrhage group,and 3-4mmol/L in cerebral infarction group separately. The HG after stroke continued about 10 days.Conclusion HG cnhanced the degree of the neural function defect and the mortality rate,and made the prognosis ofstroke worse. For decreasing the mortality rate and the degree of disability of stroke,it would be useful to limit the glucose admission,and give the patients certain dose of insulin to prevent and control HG.
