摘要
目的探讨瑞芬太尼对依托咪酯引发肌阵挛的影响。方法选择ASAⅠ~Ⅱ级择期手术患者90例,随机均分为三组:依托咪酯组(E1组)、依托咪酯泵注组(E2组)和瑞芬太尼组(R组)。E1组静注依托咪酯0.3 mg/kg,30 s内注完;E2组依托咪酯0.3 mg/kg静脉泵注[速度60μg/(kg.min)];R组依托咪酯0.3 mg/kg[速度60μg/(kg.min)]和瑞芬太尼1μg/kg[速度0.2μg/(kg.min)]同时泵注。观察肌阵挛程度并进行评分。结果 R组依托咪酯引发肌阵挛程度明显低于E1组和E2组(P<0.01),其中E2组肌阵挛的程度明显低于E1组(P<0.01)。结论瑞芬太尼能够显著降低依托咪酯引发的肌阵挛。
Objective To study the effects of remifentanil on myoclonus induced by etomidate.Methods Ninety patients were randomly allocated into three groups.Group E1: Etomidate 0.3 mg/kg was injected within 30 seconds;group E2: Etomidate 0.3 mg/kg was infused [60μg/(kg·min)];group R:Etomidate 0.3 mg/kg was infused [60μg/(kg·min)] and Remifentanil 1μg/kg was infused [0.2μg/(kg·min)] simultaneously.Myoclonus was observed after the Etomidate injection.Results The incidence of myoclonus was significantly lower in group R than that in group E1 and E2(P〈0.01),and which was significantly lower in group E2 than that in group E1(P〈0.01).Conclusion Remifentanil can reduce the incidence and degree of myoclonus induced by etomidate injection.
