钽金属棒系统治疗成人股骨头缺血性坏死

Treating avascular necrosis of the femoral head with porous coating tantalum rod system

摘要 目的评价应用多孔涂层钽金属棒系统治疗成人股骨头缺血性坏死的效果和应用价值。方法自2008年5月至2010年6月间对22例成人股骨头缺血性坏死患者施行多孔涂层钽金属棒系统固定手术治疗。病变分期中FicatⅠ期3例,FicatⅡ期19例。单侧手术17例,双侧手术5例。术前髋关节症状以疼痛和跛行为主,Harris评分平均52.5分。手术前后测量患者关节活动度的大小,并对患者进行Harris评分。术后对患者进行主观满意度调查。结果术后随访平均19.5月(2~25月)。患者切口均一期愈合,无感染、内植物松动等并发症发生。在随访期间内X线检查未发现明显股骨头塌陷及囊变区扩大等病变进展迹象,患者术前疼痛、跛行等症状缓解明显。随访时术后Harris评分为85.5分,与术前相比差异有统计学意义。主观满意度优17例,良3例,中2例,总体优良率90.9%。结论采用多孔涂层钽金属棒系统治疗FicatⅠ、Ⅱ期成人股骨头缺血性坏死可取得良好的早、中期效果。 Objective To evaluate the effect and application value of the porous coating tantalum rod system for adult avascular necrosis of femoral head.Methods Twenty-two adult patients(seventeen men,five women) with avascular necrosis of femoral head,three in stage Ⅰ and nineteen in stage Ⅱ,were received the surgery with porous coating tantalum rod system in our department between May 2008 to June 2010.The average pre-operative Harris score was 52.5.The main symptom of patients was pain in the hip joint and limp.Range of motion and Harris score were determined before and after surgery.Subjective satisfactory survey was also performed after operation.Results The average follow-up period was 19.5 months.All of the incisions were healed successfully.No complication like infection and loosen occurred.No degenerative sign like collapse and enlargement of the cysts was found within the follow-up.Preoperative symptoms like pain and limp were alleviated obviously after surgery.Harris score after the procedure was 87.5,with statistically significant difference compared with that before surgery.Result of the subjective satisfactory survey was seventeen excellent,three good and two medium.The overall excellent and good rate was 90.9%.Conclusion Treating adult avascular necrosis of femoral head in stage Ⅰ and Ⅱ with porous coating tantalum rod system can acquire good short-term and mid-term results.
