摘要
目的探讨外科综合治疗布鲁杆菌病脊柱炎的临床效果。方法手术治疗布鲁杆菌病脊柱炎26例,采用前方或后方入路进行一期病灶清除,同时根据需要进行减压、植骨、内固定治疗。术前抗布鲁杆菌药物治疗2~4周,术后常规抗布鲁杆菌治疗至少3个月,血沉和C-反应蛋白定期检查监测。结果术后切口均一期愈合,未见副损伤;发热疼痛等症状明显减轻,患者均顺利出院。随访时间6~24(10±2)个月。无复发及严重并发症发生。末次随访时X线片均显示植骨融合良好,内固定无移位。结论在长期、足量、联合应用抗布鲁杆菌药物同时,对有手术指征的布鲁杆菌病脊柱炎进行外科手术治疗是有效的。
Obiective To investigate the clinical efficacy in comprehensive surgical treatment for brucellosis spondylitis.Methods A total of 26 cases with brucellosis spondylitis were treated surgically.All of the cases underwent one-stage anterolateral or posterior debridement decompression,bone graft and instrumentation based on requirement.Anti-brucella drugs were administered 2~4 weeks before operation,routine anti-brucelliasis treatment lasted more than 3 months after operation,and blood sedimentation and C-reactive protein were monitored regularly.Results All the cases got primary incision healing,significant alleviation of fever and pain,and discharged smoothly.The follow-up period was 6~24 months(mean 10±2 months).No recurrence or severe complication was found in our cases.At final follow-up,all cases had X-ray film evidence of solid bony fusion without any instrument failure.Conclusions Based on long-term,sufficient combination anti-brucella therapy,surgical treatment for brucellosis spondylitis patients appears to be safe and effective when meeting the indications.
