CMV感染婴儿肝炎综合征患儿的肾脏损害 被引量:7

摘要 为了解CMV感染所致婴儿肝炎综合征患儿的肾脏损害,用ELISA法检测了CMV感染、非CMV感染婴儿肝炎综合征患儿和正常婴儿尿视黄醇结合蛋白(RBP),以及β_2微球蛋白(β_2MG)、N-乙酰-β-D氨基葡萄糖苷酶(NAG)、Ccr、血β_2MG等。结果表明,尿RBP和尿β_2MG均呈CMV感染患儿组>非CMV感染患儿组>对照组(P<0.001),CMV感染组71.4%、非CMV感染组44.4%尿RBP超过对照组95%参考值上限。CMV感染组84.0%尿β_2MG超过对照组上限,明显高于非CMV感染组的40.0%;尿NAG:CMV感染组、非CMV感染组分别显著高于对照组,CMV感染组68.0%、非CMV感染组60.0%超过其上限。血β_2MG:7例婴儿肝炎综合征患儿与新生儿~3岁正常婴幼儿值无显著差异。Ccr在三组间无显著差异。CMV感染与非CMV感染两组间肝功能无显著差异。CMV感染组尿RBP、β_2MG与AST正相关;NAG与γ-GT正相关。非CMV感染组β_2MG与ALP正相关。提示婴肝综合征伴有以肾小管小分子蛋白重吸收功能受损为主的肾脏损害,在CMV感染中更为严重,可能与病毒对肾脏的直、间接损害有关。 To investigate the influence of infantile hepatitis syndrome induced by CMV infection on kidneys, the urine retinal binding protein (RBP), urine β2 microglobulin (β2MG), urine NAG, Ccr and serum β2MG were determined in 43 cases with infantile hepatitis syndrome induced by CMV infection, 14 cases of infantile hepatitis syndrome no induced by CMV infection and 37 normal control cases, respectively. The results showed that urine RBP and ft>MG levels were in turn the CMV infection group>the non -CMV infection group> the control group (P<0. 001). The urine RBP, β2MG and NAG levels of 71.4% ,84. 0% and 68. 0% in the CMV infection group and of 44. 4% ,40. 0% and 60. 0% in the non - CMV infection group were all higher than the upper limit of 95% reference value in the control group, respectively. No significant! difference of seifum β2MG and Ccr were found among three groups, and of liver function did between the CMV infection and the non - CMV infection groups also. There was positive correlation among urine RBP and β2MG with AST, and NAG with 7 - GT in the CMV infection group, respectively, while urine β2MG was positively correlated with ALP in the non -CMV infection group. It is Suggested that infantile hepatitis syndrome may impair the function of kidneys, which manifest chiefly the dysfunction of tubular reabsorption, in particular more serious in children with infantile hepatitis syndrome induced by CMV infection, that may probably correlated with damage of kidneys acted by CMV directly and indirectly.
