摘要
目的探讨钢板结合形状记忆合金弓齿钉内固定治疗复杂胫骨平台骨折的临床疗效。方法 2008年6月至2010年10月收治15例复杂性胫骨平台骨折患者,其中男10例,女5例;年龄25~65岁,平均43.3岁。受伤原因：车祸伤8例,高处坠落伤5例,重物砸伤2例。按Schatzker分型为V、Ⅵ型。均采用锁定钢板结合形状记忆合金弓齿钉内固定治疗。结果 15例患者均获得随访,时间13~30个月,平均21个月。全部病例均获得骨性愈合,平均愈合时间3.2个月。按照R asm ussen胫骨髁部骨折膝关节功能评分,本组优5例,良8例,可2例,优良率86.7%。结论钢板结合形状记忆合金弓齿钉治疗复杂胫骨平台骨折,复位满意,固定牢靠,是治疗复杂胫骨平台骨折可选择的治疗方法之一。
Objective To investigate the clinical effect of Schatzker type Ⅴ and Ⅵ tibial plateau fractures with plates and shape memory alloy bow-teeth screw.Methods 15 patients with Schatzker type Ⅴ and Ⅵ tibial plateau fractures were retrospectively analyzed.There were 10 males and 5 females with an average age of 43.3 years（range 25 to 65 years）.8 cases were caused by traffic accidents,5 cases by falling from high place,2 cases by heavy objects hitting.According to the Schatzker classification,all the cases were type V and Ⅵ fractures.All the cases accepted the surgical treatment with plates and shape memory alloy bow-teeth screws fixation.Results The mean period of follow-up was 21 months（range 13 to 30 months）.All the cases obtained bone union in 3.2 months averagely.The results of clinical evaluation according to Rasmussen scores were excellent in 5 cases,good in 8 cases.Conclusion Application of plates and shape memory alloy bow-teeth screw was a superior option is helpful to satisfactory reduction and reliable fixation.It is a good choice for fixation of Schatzker type V and Ⅵ tibial plateau fractures.
