摘要
目的:探讨治疗耻骨直肠肌综合征(PCMS)型出口梗阻性便秘疗效确切的中西医结合治疗方案。方法:对顽固性耻骨直肠肌肥厚性便秘患者198例行耻骨直肠肌部分切除挂线,术后遵循个体化原则给予口服中药。结果:总有效率为100%(198/198),其中痊愈率为61.6%(122/198),显效率为21.2%(42/198),有效率为12.2%(24/198),无效率0。平均住院天数(11.2±2.0)天。术后随访2年,无便秘复发,无肛门狭窄和大便失禁。患者症状明显改善,指检肛管紧张消失,排粪造影搁架征消失。结论:耻骨直肠肌部分切除挂线加中药口服治疗耻骨直肠肌综合征型便秘具有疗效显著、痛苦小、恢复快、并发症少的特点。手术安全,简易,易于推广,中医药辨证论治,整体上调整患者的体质,恢复其正常的排便习惯。
Objective：Investigating the effective in tegraion of Chinese and western medicine treatment program to treat outlet obstruction constipation causing by PCMS.Method：198 indolent constipation patients who have hypertrophic puborectalis were treated with partial puborectalis resection and thread-drawing method,and had individual Chinese medicine everyday after operation.Result：The total effective rate was 100%（198/198）,in which cure rate was 61.6%（122/198）,markedly effective rate was 21.2%（42/198）,effective rate was 12.2%（24/198）,ineffective rate was 0.Average（11.2±2.0）days.Patients were followed up for 2 years,there were no recurrence of constipation,no anal stenosis and fecal incontinence.Symptoms of patients improved significantly.Anal tension and shelf symptoms disappeared.Conclusion：The results show that partial puborectalis resection and thread-drawing method along with oral Chinese medicine has a significant effect on PCMS constipation.There is no pain,quicker recovery and fewer complications.Integrative Chinese and western medicine treatment program is effective,safe,simple and easy to spread.Traditional Chinese medicine differential treatment overall adjust the patients＇ system to restore normal bowel habits.It is effective.
