颈椎小关节部分破坏后钢板固定的扭转强度

摘要 目的:了解颈椎小关节双侧部分切除后颈椎后路钢板固定的扭转强度。方法:12 例颈椎标本(C4~7)C5~6 关节内侧切除50 % ,6 例为对照组,6 例钢板固定,测试扭转强度。结果:对照组最大扭矩17 .83 ±2 .16 N·m ,钢板组最大扭矩22 .78 ±4 .75 N·m ,其差异有显著性。对照组3 例小关节突骨折,钢板组5 例螺钉孔与关节破坏边缘处发生骨折。结论:颈椎后路钢板固定可显著增大扭转强度,但螺钉孔处骨折对钢板固定强度产生较大影响。螺钉固定位置须与结构破坏部分保持3 m m 以上距离,以免强度减弱。 To study torsion strength of posterior plate fixation for cervical spine after 50% of facetectomies. Methods:Twelve adult human cervical specimens(C 5~6 )were factectomied 50%, six of which were fixed with plate soon afterwards. All specimens were tested with torsion strength and observed the anatomical destructions. Results:Maximum torsion loads were 17.83±2.16 N·m after half bilateral facetectomies and 22.78±4.75 N·m following plate fixation. There was a significant difference between the two groups. Facet joint fracture occurred in three specimens of contrast group,and fracture did through the screw holes and at the edge of damaged joint in five of plate group.Conclusions:Although the torsion strength of plate was in creased significantly after half bilateral facetectomies, whose maximum torsion load was still less than the that of normal specimen. The failure occurred at 22.78 N·m torsion load with fracture through the screw holes and at the edge of damaged joint to avoid torsion strength decreasing,a distance no less than 3 mm must be remained between screw hole and edge of destroyed facet joint.
