摘要
目的观察类风湿关节炎患者血清和关节滑液中Ghrelin和TNF-α水平,探讨二者在RA关节滑膜炎性病变中的免疫调节作用。方法采用酶免疫分析方法检测36例活动期RA患者与36例正常对照人群血清和关节滑液中Ghrelin和TNF-α水平,分析二者的相关性。结果活动期RA患者血清Ghrelin水平显著低于正常对照组(P<0.05),而TNF-α水平明显高于正常对照组(P<0.01);RA患者血清中Ghrelin与TNF-α水平无显著相关性(r=0.364,P>0.05)。活动期RA患者关节滑液Ghrelin和TNF-α水平均显著高于血清(P<0.05),并且Ghrelin与TNF-α水平呈负相关(r=0.819,P<0.05)。结论 RA患者血清中Ghrelin水平降低,TNF-α水平升高;关节滑液中Ghrelin和TNF-α水平高于血清,且滑液中Ghrelin与TNF-α呈显著负相关。提示Ghrelin可能与TNF-α共同参与RA的发病过程。
Objective To observe the alterations of ghrelin and TNF- α levels in serum and articular synovial fluid from patients with rheumatoid arthritis,and to explore their immunoregulatory effects on the articular synovial membrane inflammation of RA. Methods The serum and synovial fluid were obtained from 36 patients with active RA and from 56 healthy controls,and levels of ghrelin and TNF- α in serum and synovial fluid were measured by enzyme immunoassay. The correlation between ghrelin and leptin levels in serum and SF of RA patients was calculated. Results The level of ghrelin in serum of patients with RA was significantly lowered compared with the control group (P 〈 0.05),and the levels of TNF- α in serum of patients with RA were significantly increased compared with the control group (P 〈 0.01). There was no correlation between serum levels of ghrelin and TNF- α in RA patients (r=0.364,P 〉 0.05). Both the levels of ghrelin and TNF- oL in articular synovial fluid were higher than those in serum of patients with active RA (P 〈 0.05). And the Pearson's correlation analysis showed that articular synovial fluid ghrelin levels was negatively correlated with that of TNF- α (r=0.819,P 〈 0.05). Conclusion Serum ghrelin levels decreased and TNF- α levels increased in patients with active RA,and levels of ghrelin and TNF- α in articular synovial fluid were higher than those in serum of patients with active RA. There has negative correlation between the levels of ghrelin and TNF- α in articular synovial fluid of patients with RA. These results suggested that ghrelin may participate,together with TNF- α ,in the pathogenesis of RA.
