摘要
目的 研究耳内式助听器的特性。方法 应用GSI - 16纯音听力计附声场测听系统 ,观察 16 8耳不同类型助听器使用者助听前的听力损失和助听后的听力补偿 ;测试裸耳及佩戴不同类型耳内式助听器后外耳道共振峰幅值 ;测试 2 2耳耳内式助听器使用者的言语分辨率。结果 耳内式助听器 10 0 0~ 40 0 0Hz的听力补偿优于2 5 0~ 5 0 0Hz的听力补偿 (P <0 .0 1) ;46耳裸耳及佩戴助听器后外耳道共振峰值分别为 2 92 9Hz及 3 0 16Hz,两者无明显差异 (P <0 .0 5 ) ,ITE、ITC和CIC在助听器无输出的情况下由于耳廓的集声和滤波作用在其麦克风口声增益分别为 6 .2 9、8.0 8和 9.0 1dBSPL ,三者有极显著性差异 (P <0 .0 1) ;开放法、闭合法 2 2耳言语测试显示助听器使用者助听前后及对单音节词和双音节词的分辨率有极显著性差异 (P <0 .0 1) .结论 耳内式助听器的听力补偿高频优于低频 ,对外耳道的共振峰值影响较少 。
Objective To study the characteristics of in-the-ear(ITE),in-the-canal(ITC) and completely-in-the-canal(CIC) hearing aids.Methods The GSI-16 clinical two channel audiometer with built in sound field system was used to observe 143 cases(168 ears)with various hearing aids.Their hearing loss and hearing compensation were tested before and after the aids being used.The influence of hearing aids to the resonance peak value of the external auditory canals in the ears without or with hearing aids was tested.The speech discrimination rates of the 22 cases with hearing aids were tested with the standards and methods of acoustic-speech resume assessment.Results Hearing compensation at 1 000~4 000 Hz was prior to that at 250~500 Hz(P<0.01).The average resonance peak values of the external auditory meatus of the 46 ears without or with hearing aids were 2 929 Hz and 3 016 Hz respectively.There was no singnificant difference between them(P<0.05).If the ITE,ITC and CIC hearing aids were not turned on,the average acoustic gains at microphone opening on account of collector and filter of auricle were 6.29,8.08 and 9.01 dB SPL respectively.There were very significant differences among them(P<0.01).The speech testing with opening and closing methods showed that there were very significant difference(P<0.01) in 22 ears with or without the hearing aids and for monosy llables or disyllables.Conclusion Th hearing compensation of high frequency is better than that of low frequency in cases with hearing aids.The influence to the resonance peak values of the external auditory canals is mild.Speech discrimination rate can be raised.
