摘要
目的观察联合应用乌司他丁(UTI)和甘油果糖对大鼠脑出血后水通道蛋白4(AQP4)的影响。方法 100只SD大鼠随机均分为对照组(A组)、脑出血组(B组)、乌司他丁组(C组)、甘油果糖联合乌司他丁组(D组)。采用自体动脉血注入尾状核法制成大鼠脑出血模型。A组不注药,C组尾静脉注入乌司他丁1.5 U·g^(-1)·h^(-1),D组尾静脉注入甘油果糖0.6 ml·kg^(-1)·h^(-1)和乌司他丁1.5 U·g^(-1)·h^(-1),B组在相同时间内给予等量生理盐水,各组分别在造模后6 h、1、3、5、7 d免疫组化法观察AQP4表达,伊文思蓝法测血脑屏障(BBB)的通透性,干湿重法计算脑含水量。结果与A组相比,B、C、D组AQP4于造模后6 h开始升高,3 d达到高峰,7 d仍高于正常,D组明显低于B组(P<0.05);脑出血后BBB通透性均在出血后6 h开始增高,1~3 d最高,随后逐渐降低,D组最低(P<0.05);B组脑含水量6 h开始增加,3 d达高峰后缓慢下降,7 d仍高于正常(P<0.05),D组各时点含水量均低于C组(P<0.05)。结论脑出血后可能通过上调APQ4表达,增加BBB通透性,参与脑水肿形成,而早期联合给予甘油果糖与乌司他丁可明显抑制此进展。
Objective To observe the effect of combined use of ulinastatin and fructose glycerol on a AQP4 fter experimental cerebral hemorrhage (ICH) inrats. Methods One hundred SD rats were randomly divided into control group (group A), intracerebral hemorrhage (group B), ulinastatin group (group C), glycerol fructose plus ulinastatin group (group D). The ICH models were established by stereotaxic injection of autologous artery blood into the eaudate nucleus. Ulinastatin 1.5 U.g-1 h-1 was given to the UTI rats, glycerol fructose 0.6 ml-kg-1h-1 ulinastatin plus 1.5 U.g -1h1 were given to Group D, group B received normal saline at the same time. The expression of AQP4 was observed at 6 h, 1, 3, 5 and 7 d, respectively with immunohistochemistry. Blood-brain barrier (BBB) permeability was detected with Evans blue staining, and the ratio of wet and dry brain was calculated. Results Compared with group A, AQP4 began to increase at 6 h, peaked at 3 d, and was still higher than the normal at 7 d; and group D was significantly lower in AQP4 expression than group B (P〈0.05). BBB permeability also began increase at 6 h after hemorrhage, topped during 1- 3 d and then gradually decreased. The brain water content in group B began increase at 6 h; peaked also at 3d and then decrease but still higher than the normal (P〈0. 05) ; Group D had the lowest water content (P〈0. 05). Conclusion The formation of brain edema after cerebral hemorrhage may result from the upregulation of APQ4 and increase in BBB permeability, but glycerol fructose combined ulinastatin can inhibit this process.
