摘要
目的探讨肝功能不良的影像学可切除肝癌的二期手术治疗方法。方法回顾性分析近期影像学可切除肝癌因肝功能不良而行二期手术治疗的病例资料10例。术前先行肝动脉化疗栓塞(TACE)治疗,同时加强保肝、抗病毒治疗,待肝功能好转后,再行二期手术切除。结果治疗组均成功切除肿瘤,无手术死亡,术后未发生肝功能衰竭。结论对于肝功能不良的影像学可切除肝癌患者,可先行TACE治疗,经积极保肝、抗病毒治疗后,可获得二期手术切除的机会,且效果良好。
Objective To probe the therapy for the secondary hepatectomy of imageologically resectable hepatocellular carcinoma with poor liver function.Methods Clinical data of recent 10 hepatocellular carcinoma patients who underwent secondary hepatectomy were analyzed retrospectively.TACE combined with liver protection and antiviral therapy was performed before tumor resection because of poor liver function.All patients then were undergone secondary tumor resection when their liver function recovered.Results All patients’tumors were removed successfully.There was no perioperative mortality and no postoperative liver failure.Conclusion Combined with liver protection and antiviral therapy,TACE is performed to these patients with resectable liver tumor but poor liver function.Then secondary surgical treatment can be done to some of these patients and can obtain good results.
