摘要 目的:了解儿童血液病化疗并发感染血培养病原菌分布及耐药性,为临床合理使用抗菌药物、有效控制感染提供依据。方法:回顾分析2008年3月至2011年8月我科住院血液病化疗并发感染患儿血培养和药敏试验结果。结果:共分离出病原菌30株,其中G-菌17株占56.6%,G+菌11株占36.7%,真菌2株占6.7%;哌拉西林/他唑巴坦、亚胺培南对G-菌耐药率低;万古霉素和利奈唑胺对G+菌的耐药率低。结论:血液病患儿化疗并发感染血培养病原菌以G-杆菌为主,临床上在细菌培养结果出来之前建议使用以G-菌为主、兼顾G+菌的广谱抗菌活性强的抗菌药物。 Objective:To investigate the distribution of pathogenic bacteria in blood culture of chemotherapy with infection in children with hematologic malignancies and to observe the antibiotic resistance of these bacteria in order to provide the reference for clinical antimicrobial agents usage.Methods:The patients who had positive blood culture from Mar 2008 to Aug 2011 were studied retrospectively by antimicrobial susceptibility results of pathogens isolated from bloodstream.Results:Among the total of 30 isolates of pathogens,the Gram-negative bacilli were 56.6%,the Gram-positive 36.7% and the fungi were 6.7%.Piperacillin-tazobactam,imipenem,vancomycin and linezolid were all with low resistance rate.Conclusions:The Gram-negative bacteria are the main pathogens of chemotherapy with infection in childhood hematologic malignancies.So in clinics before identification of bacteria it is better to use the antibiotics against the Gram-negative bacteria,and the Gram-positive bacteria.
