摘要
目的 :探讨缺血性心脏病患者左室功能低下与室性心律失常数量、室性期前收缩 (室早 )严重程度之间的相关性。方法 :将 118例缺血性心脏病患者按照左室射血分数 (L VEF)值和左室舒张末期容积 (VD)值分组 ,比较不同组别与室早数量、室早程度的关系。结果 :L VEF≤ 0 .30、L VEF0 .31～ 0 .40组及 L VEF≥ 0 .41组间在室早数量上无显著性差异 (P>0 .0 5 ) ;而 3组在 L own 级以上室早者 (分别为 5 5 .6 %、38.2 %和 2 0 .0 % )间均存在显著性差异 (P均 <0 .0 5 ) ;L VEF≤ 0 .30组中 VD≥ 12 0 m l者与 VD<12 0 ml者在 L own 级以上室性心律失常者分别为 88.9%与 44 .4% ,存在显著性差异 ;L VEF与 L own分级呈高度负相关 (r=- 0 .92 )。结论 :缺血性心脏病 L VEF值与室早 L own分级呈负相关 ,发生严重心律失常比例随心功能低下而增多。心功能低下时则心脏扩大 ,严重心律失常发生率越高 。
Objective:To study the correlation between left ventricle hypofunction and the frequency and(or) the severity of the ventricular arrhythmias in ischemic heart disease.Methods:118 patients with ischemic heart disease were included in this study.The patients were divided into different groups according to left ventricular ejection fraction(LVEF) figures and left ventricular enddiastolic volume(V D) figures.Comparison of the frequency and the severity of the ventricular arrhythmias among different groups were performed.Results:There was no significant difference in the frequency of ventricular arrhythmias among the groups of LVEF≤0 30,LVEF between 0 310 40,and LVEF≥0 41.However,there was remarkable difference in the degree of ventricular premature beat above Lown′s scale Ⅲ among the different groups(all P <0 05).In the group of LVEF≤0 30,there was remarkable difference in the frequency of ventricular arrhythmias with Lown′s scale above Ⅲ between the group of V D≥120 ml and the group of V D<120 ml.There was a significant negative correlation between LVEF and Lown′s scale ( r = -0 92 ).Conclusions:Negative correlation exists between the LVEF figures of ischemic heart disease and the Lown′s scales of the ventricular premature beat.The more severe the cardiovascular hypofunction,the more frequent the incidence of severe arrhythmias.When the heart hypofunction is complicated with cardiectasis,the occurrence of severe arrhythmias are significantly increased and the prognosis is unfavorable.
