摘要
:[目的]研究紫杉醇治疗耐药性妇科恶性肿瘤的疗效和毒性反应。[方法]紫杉醇治疗88例经铂类化疗失败的卵巢癌、输卵管癌、子宫内膜癌和绒癌。[结果]完全缓解11例 ,部分缓解32例 ,总有效率为48 9%(43/88)。对卵巢癌、输卵管癌、子宫内膜癌和绒癌的有效率分别为42 6%(29/68)、66 7%(6/9)、85 7%(6/7)和50 0%(2/4)。毒性反应有白细胞总数和中性粒细胞下降 ,Ⅲ、Ⅳ度毒性反应分别为27 3%和34 1%。超敏反应有面部潮红(27%)和皮疹(3 4%) ,外周神经毒性占63 6% ,脱发占100%。胃肠反应和心、肝、肾毒性均不严重。[结论]紫杉醇对耐铂类的卵巢癌、输卵管癌、子宫内膜癌和绒癌均有较好疗效 。
To determine the efficacy and toxicity of paclitaxel in drug resistant malignant gynecologic cancer From Sep 1993 to Dec 1999,88 patients with platinum refractory ovarian cancer,endometrium carcinoma and chroriocarcinoma were treated with paclitaxel,175mg/m2 alone or 135mg/m2 in combination with other anticancer drugs,intravenously over 3 hours every 3 weeks Among 88 assessable patients,complete response were observed in 11(12 5%)and partial response in 32 patients(36 4%)with an overall response rate of 48 9% Sever(grade 3 or 4)leukopenia and neutropenia were seen in 27 3% and 34 1% respectively Peripheral neurologic(63 6%)and alopecia(100%)were common A part from flushing of face during paclitaxel infusion in 24 patients and skin rash in 3 patients,the gastrointestinal,cardiac,liver and renal toxicities were mild [Conclusion]Paclitaxel is considered to be a drug of choice in the treatment of patients with gynecologic cancer who are resistant to conventional chemotherapy
