长柄双动股骨头置换治疗高龄股骨粗隆间骨折的体会 被引量：3

摘要 目的:探讨长柄型双动股骨头置换治疗老年骨质疏松患者粗隆间粉碎性骨折的临床疗效。方法:笔者采用人工股骨头置换治疗高龄股骨转子间骨折15例,骨折分类按Evans分型,Ⅲa型7例,Ⅲb型5例,Ⅳ型3例。取后外侧切口,术中注意保留大小粗隆部位的骨折块,争取将其重新复位,用钢丝固定,安放人工股骨头。结果:15例术后均获8～24个月随访,平均16.2个月。住院时间平均17d,术后14例31d完全负重行走,1例因其他内科疾患扶拐行走。结论:该手术方法具有术后短期即可下地行走、髋关节功能接近或恢复到受伤前水平、缩短患者卧床时间、更符合生物力学、尽快恢复功能的特点,是目前较好的治疗方法。
