重组人生长激素治疗青春期前特发性矮小症60例疗效分析 被引量:10

Analysis of the Efficacy of Recombinant Human Growth Hormone for the Treatment of Idiopathic Short Stature Before Puberty

摘要 【目的】探讨重组人生长激素(recombinant human growth hormone,rhGH)对青春期前特发性矮小症(Idiopathic short stature,ISS)儿童促身高增长的疗效。【方法】经胰岛素和左旋多巴药物联合激发试验测定生长激素(GH)确诊缺乏的ISS儿童60例,每晚睡前皮下注射rhGH治疗0.15IU/(kg.d),疗程6个月,并对其疗效进行一年的观察。【结果】ISS患儿经生长激素治疗后,生长速率(.growt h velocity,GV)由治疗前测量的(3.61士0.42)cm/年提高到治疗后(7.69±3.31)cm/年,差异有显著性(P〈0.05)。身高由治疗前(134.12±14.14)cm提高到治疗后(141.20±13.88)cm,差异有显著性(P〈0.05)。治疗期间除少数肝功能轻度异常,有6例儿童在注射部位出现一过性红肿,双下肢轻度水肿和过敏性皮疹等,但未予处理后自行消失,另未发现其他明显副作用。【结论】皮下注射rhGH对治疗儿童ISS有增快生长速度作用,疗效肯定,是一种安全有效的方法,值得儿科临床推广应用。 [Objective]To explore the effect of gene recombinant human growth hormone(rhGH) for improving the growth in stature in children with idiopathic short stature(ISS). [Methods] Sixty ISS children diagnosed by provocative test with insulin and levodopa for measuring growth hormone(GH) deficiency were given subcutaneous injection of rhGH 0.15IU/(kg. d) before sleep every night for 6 months. The efficacy was observed for a year. [ResultsiAfter GH treatment for ISS children, the growth velocity(GV) increased from (3.61±0.42) cm/year before treatment to (7.69 ±3.31 )cm/year after treatment, and there was significant difference( P〈0.05). The body height also increased from ( 134.12±14.14 ) cm before treatment to ( 141.20 ±13.88 )cm after treatment, and there was significant difference( P〈0.05). During the treatment, a few patients had mildly abnormal liver function, and 6 patients had transient flare in the injection site, mild edema of both lower limbs and allergic skin rash, and these symptoms disappeared without treatment. Other obvious side effects were found. [Conclusion] rhGH subcutaneous injection for the treatment of ISS children can improve the GV and has certain efficacy. Therefore it is a safe and effective method, and worthy of clinical applieation in pediatrics.
