摘要
目的通过研究叶天士对《伤寒论》黄芩汤方证的拓展运用,以管窥叶天士对《伤寒论》的认识与证治发挥。方法采用自行研发的"医案查询统计分析系统,对《临证指南医案》中有关黄芩汤的运用从症状、病机、方药等方面,采用频次计数法挖掘整理。结果符合检索要求的医案71例,涉及症状90种,主要为纳呆、痢、疟、泄泻、呕吐、腹痛等;病机出现总频次为114次,主要为热内结、少阳胆热、肝胆相火、火郁胆胃胸脘等;与黄芩、芍药配伍的药物115种,主要为黄连、人参、干姜、枳实、半夏、陈皮等。结论叶天士在《临证指南医案》中运用黄芩汤治疗病机为少阳气分里热的痢疾、泄泻、疟疾等病证。为《伤寒论》中以胆火上炎、里热为甚的少阳病证的治疗指明以清泄里热为主,兼以顾护阴液的治疗方向。
Objective To research cognition and exertion of sndrome and treatment of Huangqin Tang in Shanghan Lun by Ye Tianshi.Methods Using the database of Analysis System of Medical Records,this article not only analyses the symptom,pathogenesis,treat method and medicine of Linzheng Zhinan Yi＇an,but adopt frequency counting to excavate.Results Medical records in keeping with the requirements of the retrieval were 71 cases,90 kinds of symptoms were related,mainly were anorexia,diarrhea,malaria,diarrhea,vomiting,abdominal pain,etc.114 kinds of pathogenesis were related,mainly were endogenous heat,gallbladder hot,etc.115 kinds of drugs were applied with Huangqin and Shaoyao,mainly were Huanglian,Renshen,Ganjiang,Zhishi,Banxia,Chenpi,etc.Conclusion Ye Tianshi expanded the application of Huangqin Tang into spleen-stomach syndrome and malaria,which possess the pathogenesis of shaoyang gallbladder-heat,gallbladder-wood attacking spleen-stomach,liver-wood attacking spleen-stomach and fire-stagnation in gallbladder-stomach or chest-epigastrium.
