神经节苷脂对重型颅脑创伤颅高压减压术后的疗效观察 被引量：11

The efficacy of gangliosides on the situation after decompression surgery of severe traumatic brain injury induced intracranial hypertension

摘要 目的观察神经节苷脂对重型颅脑创伤颅高压大骨瓣减压术后的疗效。方法重型颅脑创伤颅高压行减压术患者60例,随机分为研究组(30例)和对照组(30例),研究组在对照组常规治疗的基础上给予神经节苷脂,观察治疗前、治疗2周后的颅内压、脑水肿、脑梗死、意识清醒情况、血浆炎性因子水平,治疗2周后的双侧大脑中动脉血流情况,并于治疗后3个月评价日常生活活动能力和认知功能。结果研究组治疗后的意识清醒人数多于对照组(P<0.05),中、重度脑水肿度、脑梗死人数低于对照组,但颅内压没有差异;研究组的双侧大脑中动脉血流情况优于对照组(除Vmax外),如Vmin、Vmean上升,PI、RI降低;研究组血浆中的IL-6、hs-CRP、TNF-α水平均低于对照组;治疗后3个月,研究组的巴氏指数(P<0.01)和MMSE(P<0.05)均高于对照组。结论神经节苷脂可改善重型颅脑创伤颅高压减压术后的近期和远期症状,可在颅脑创伤颅高压减压术后治疗中推广。
