摘要
目的 探讨单侧唇裂继发鼻畸形的理想整复方案.方法 采用蝶形切口入路,充分解剖复位,自体肋软骨修整成形后植入两鼻翼软骨内侧脚间重构外鼻支撑、矫正鼻中隔畸形,植入穹窿部加强鼻翼,必要时垫高鼻翼基底矫正鼻翼基底下陷,一次性完成对复杂鼻畸形的整复.结果 患侧鼻翼塌陷、鼻翼基底过低、鼻尖不正、鼻孔不对称等明显改善,鼻孔基底部缺裂及上唇瘢痕有较大改善.结论 自体肋软骨用于纠正单侧唇裂继发鼻畸形效果可靠,鼻畸形形态修复满意.
Objective To explore an effective treatment in repairing secondary unilateral cleft lip nasal defurmity. Methods On the basis of sufficient anatomical reset,the autologous costal cartilage were implanted to recnnstruetive the nasal septum,the lower lateral cartilage to supply the powerful supporting strength for nasal constructions by butterfly incision. When neccssry,the low alar base was elevated. Results Collapsed nasal alar,the low alar base,oblique nnse tip and nostril asymmetry got significantly improved,the separated nostril base and the upper lip scar got greatly improved. Conclusion Autologous costal cartilage can effectively correct nasal deformity of unilateral cleft lip in secondary rhinoplasties,and the outcome is excellent.
