摘要
目的 研究吸入激素对非急性发作期哮喘患者肺功能及血皮质醇的影响。方法 测定 2 2例非急性发作期哮喘患者吸入中等量皮质激素BUD四周前后的FEV1% ,PEFR(L/min)及用峰速仪监测呼主峰流速 (PEF ,L/min) ,同时测定血皮质醇及咽拭子培养进行治疗前后比较。结果 治疗后FEV1由 5 6 .77± 11.79%增至 6 5 .89± 11.97% ,PEFR由 4.40± 1.6 9L/min增至 5 .6 8± 1.71L/min ,PEFRV由 14.2 2± 9.99%降至 4.82± 4.6 2 % ,症状记分由 1.99± 0 .6 9降至 1.19± 0 .92 ,经t检验有差异 (P <0 .0 1或P <0 .0 5 )。而血皮质醇由 14.92± 5 .99μg/dl降为 13.91± 5 .6 2 μg/dl,但无统计学上意义 (P >0 .0 5 ) ,咽拭子培养均为阴性。临床控制率 72 .7% (16 / 2 2 ) ,显效率 18.2 % (4/ 2 2 ) ,总有效率 90 .9%。结论 (1)中等量皮质激素吸入是目前哮喘治疗的推荐方法 ,而BUD最好。(2 )肺功能自身监测是指导用药和管理哮喘的最有效方法。
Objectives To investigate the effect of inhaled glucocorticoid aerosol on pulmonary function and serum cortisol for patients with asthma on no attacks episode.Methods FEV1%,PEFR(L/min),serum cortisol (μg/dl) and mycete of throat cultures were measured in 22 patients with asthma on no attacks episode .After having inhaled glucocorticoid aerosol for 4 weeks,the same indices were measured.And PEF was measured in every morning and evening by Peak Flow meter.Results Mean values of FEVl,PEFR,PEFRv,symptom mark were 56.77±11.79%,4.40±1.69L/min,14.22±9.99%,1.99±0.69in patients respectively before treatment.After treatment they were 65.89±11.97%,5.68±1.71L/min,4.82±4.62%,1.19±0.92.There were not significant in statistics(P<0.01 or P<0.05).After treatment serum cortisol was 14.92±5.99μg/dl,which was higher than that before treatment(13.91±5.62μg/dl) in patients.There was not significant in statistics(P>0.05).Mycete of throat cultures were negative in patients before treatment and after treatment.Clinical control rate was 72.7%(16/22),significant effect rate was 18.2%(4/22),total effect rate was 90.9%.Conclusion It suggested that the medium dose inhaled glucocorticoid aerosol(e.g.Budesonide) should be introduced recently.It is the best method that patients should measure pulmonary function themselves for control and treatment in asthma.
