摘要 目的:探讨99mTc-DTPA与99mTc-EC两种核素肾动态显像对慢性肾脏病患者肾功能判断的临床价值。方法:分别对20例正常对照组、70例慢性肾脏病肌酐正常组和48例慢性肾脏病肌酐升高组进行两种显像剂的肾动态显像,并测定肾小球滤过率(GFR)和肾有效血浆流量(ERPF)。结果:与正常组相比,慢性肾脏病肌酐正常组的GFR值降低,差异有统计学意义(P<0.01),ERPF值降低,差异无统计学意义(P>0.05),GFR/ERPF值降低,差异有统计学意义(P<0.05);与正常对照组相比,慢性肾脏病肌酐升高组的GFR值降低,差异有统计学意义(P<0.01),ERPF值降低,差异有统计学意义(P<0.01),GFR/ERPF值降低,差异无统计学意义(P>0.05)。3组的GFR与ERPF均呈正相关,相关性依次为r=0.647、r=0.794、r=0.863。结论:对慢性肾脏病患者,GFR是评价其肾功能早期变化的敏感指标,同时检测GFR和ERPF可对其肾功能受损程度提供更全面的信息。GFR与ERPF有良好的相关性。 Objective:To assess the value of dynamic renography by 99mTc-DTPA and 99mTc-EC in evaluating the renal function of patients with Chronic Kidney Disease(CKD).Method:Dynamic renography were performed on 20 normal cases,70 cases with CKD patients who have normal serum creatinine(Scr),and 48 CKD cases with abnormal serum creatinine.The Glomerular Filtration Rate(GFR) and Effective Renal Plasma Flow(ERPF) were analyzed.Results:Compared with the normal group,GFR and GFR/ERPF of the group CKD patients with normal serum creatinine were lower.There was significant difference among two groups(P<0.01,P<0.05),and ERPF decreases,the difference is not statistically significant(P>0.05);Compared with the normal group,GFR and ERPF of the group CKD patients with abnormal serum creatinine decreased.There was significant difference among two groups.(P<0.01,P<0.01).GFR/ERPF decreased,the difference is not statistically significant(P>0.05).ERPF was positive correlation with GFR in three group,and the correlation coefficient were 0.647、0.794、0.863.Conclusions:For Chronic kidney disease,GFR is a sensitive method in evaluating the changes of early renal failure and simultaneously detecting GFR and ERPF can provide more comprehensive information.GFR and ERPF have good correlation.
