摘要
目的:为了探讨不同类型肿瘤患者的血脂和肿瘤标志物水平的临床研究。方法:测定了31例胰腺癌、38例乳腺癌和36例卵巢癌的血脂水平,包括总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)和脂蛋白α[Lp(α)]的水平。化学发光免疫分析了胰腺癌、乳腺癌和卵巢癌的肿瘤标志物CA19-9、CA15-3和CA125的水平,并与66例正常对照组进行了对比性分析。结果:在血脂分析中,31例胰腺癌、38例乳腺癌和36例卵巢癌的血清TC和HDL-C水平较之66例正常对照组明显降低(P<0.05~<0.01),血清TG和LDL-C水平稍降低,无明显差异(p均>0.05),血清Lp(α)水平明显增高(P均<0.01)。在肿瘤标志物分析中,31例胰腺癌患者的血清CA19-9水平较之66例正常对照组明显增高(P<0.01),阳性率83.8%,CA19-9水平与CA15-3和CA125的相关系数分别为0.2367和0.3145(P<0.01)。38例乳腺癌患者血清CA15-3水平和36例卵巢癌患者血清CA125水平亦明显增高(P均<0.01),阳性率分别为84.2%和80.6%,乳腺癌患者血清CA15-3与CA19-9和CA125的相关系数分别为0.8092和0.5744(P均<0.01),卵巢癌患者的血清CA125水平与CA19-9和CA15-3水平的相关系数分别为0.3416和0.4582(P均<0.01)。结论:不同类型肿瘤患者都存在着血脂代谢紊乱的现象,肿瘤标志物血清CA19-9、CA15-3和CA125是诊断胰腺癌、乳腺癌和卵巢癌患者的最佳指标。
Objective To explore the clinical study in determining blood lipid and tomor markers in patients with different types cancer. Methods Blood lipid (with biochemistry) and tomor markers (CAlg-9,CA15-3 and CA125) (with say) levels were determined in 31 patients with pancreatic carcinoma, in 38 patients with breast cancer and in 36 patients with carcino- ma of the ovary and 66 normal controls. Also, made a comparative Analysis. Results In blood lipid analysis, serum TC and HDL-C levels were significantly lower in 31 patients with pancreatic carcinoma, in 38 patients with breast cancer and in 36 patients with carci- noma of the ovary than those in 66 controls (P 〈 0.05 -〈 0.01 ). The serum TG and LDL-C levels were minor declined, but no signif- icant difference (P 〉0.05). The serum Lp(or) level was distinctly higher(P all 〈0.01 ). In tomor markers analysis, serum CA19-9level was apparently higher in 31 patients with pancreatic carcinoma than that in 66 controls (P 〈0.01 ), positive rate was 83.8%, the correlation coefficient between CA19-9 level and CA15-3 ,CA125 was 0.2367 and 0.3145 (P 〈0.01 ). Serum CA15-3 and CA125 lev- els were significantly higher in 38 patients with breast cancer and 36 paitents with carcinoma of the ovary than those in 66 controls (P all 〈0.01), positive rate were 84.2% and 80.6% respectively. The correlation coefficients between CA15-3 and CA19-9, CA125 level in 38 patients with breast cancer were 0. 8092 and 0. 5744 ( P all 〈 0.01 ). The correlation coefficients between CA125 and CA19-9, CA15-3 levels in 36 patients with carcinoma of the ovary were 0.3461 and 0.4582 separately (P all 〈 0.01 ). Conclusion There were blood lipid metabolic disorder in patients with different types of cancer. The tumor markers of CA19-9, CA15-3 and CA125 were valuable for diagnosis of patients with pancreatic carcinoma, breast cancer and carcinoma of the ovary.
