摘要
目的分析我院静脉药物配置过程中常见不合理用药情况,并提出不合理用药的防范措施。方法对我院2010年5月至2011年5月静脉药物配置过程中药师遇到的不合理用药医嘱进行统计分析。结果共审查出不合理用药医嘱213例(占0.36%),主要包括给药方法、配伍禁忌、给药剂量、给药浓度、溶媒选择以及电脑录入等方面的问题。结论药师通过静脉药物配置及时发现并纠正临床用药不合理现象,有效减少了临床因用药不合理引起的不良事件,充分发挥了药师的价值,促进患者合理用药。
Objective To analyze statistically the common unreasonable drug use in the intravenous disposition of our hospital, and puts forward the preventive measures. Methods The unreasonable drug use prescriptions in our hospital were summarized by pharmacists and analyzed statistically from May 2010 to May 2011. Results 213 （0. 36% ） cases of unreasonable drug use prescriptions were examined, mainly including administration, incompatibility, doses, concentration, solvent selection and computer input process. Conclusion Pharmacists can detect and rectify clinical unreasonable drug use in advance. The incident caused by clinical unreasonable use will be decreased, and pharmacists play an important role in checking the intravenous disposi- tion and promoting rational drug usage for patients.
