气管与支气管支架置入术的麻醉处理 被引量:1

摘要 为了观察压缩雾化吸入表面麻醉在气管、支气管支架置入术中的临床效果,对85例恶性肿瘤合并气管、支气管狭窄或食管气管瘘的患者,使用德国百瑞"压缩雾化吸入器"在张口自主呼吸方式下吸入1%地卡因30~40mg,充分进行口腔、咽喉、气管、支气管黏膜表面麻醉后即行经口喉镜明视下国产MTN型NiTi形状记忆合金气管或支气管支架置入术。所有患者均能安静合作,一次性成功置入气管或支气管支架,生命体征明显改善,呼吸困难症状立即缓解,脉搏血氧饱和度明显增高。初步研究结果提示,压缩雾化吸入表面麻醉方法可用于气管或支气管支架置入术,操作简便,安全可靠。 The objective of this study was to observe the clinical efficacy of tracheobronchial stent insertion under compressed nebulized inhalation anesthesia.Eighty-five patients with cancer induced tracheal stenosis or esophago-tracheal fistula were enrolled.After nebilized inhalation of 1% dicaine(30-40 mg) via compressed nebulizer,all patients were treated with nickel-titanium shape memory stents through oral cavity under the help of direct vision laryngoscope.All patients could quietly cooperate and the placement of tracheobronchial stent were successful one time of all 85 patients.The symptom of dyspnea relieved immediately after the procedure and pulse oximetry(SpO2) gradually increased.In conclusion,the data of our preliminary study suggests that compressed nebulized inhalation anesthesia is safe and reliable for tracheobronchial stent insertion.
