血浆网膜素-1在2型糖尿病合并缺血性心脏病中的变化及意义

Change and significance of plasma omentin-1 in type 2 diabetes mellitus combined with ischemic heart disease

摘要 目的测定2型糖尿病(T2DM)伴缺血性心脏病患者血浆网膜素-1(Omentin—1)水平,探讨血浆Omentin-1与2型糖尿病并发缺血性心脏病的关系。方法收集70例男性2型糖尿病患者,按照是否伴有缺血性心脏病分为单纯2型糖尿病组36例(T2DM组)、2型糖尿病合并缺血性心脏病(IHD)组34例(T2DM+IHD组)。另选取年龄相匹配的39例健康男性为对照组(NGT组)。三组均统计年龄、体质量指数(BMI)、空腹血糖(FBG)、糖化血红蛋白(HbA1C)、血清空腹胰岛素(Fins)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL—C)、低密度脂蛋白胆固醇(LDL—C)、c-反应蛋白(CRP)及血浆Omentin—1、白细胞介素-6(IL-6)水平,计算LDIVHDL,并用稳态模型计算胰岛素抵抗指数(HOMA—IR)。应用SPSS17.0软件进行数据处理,组间比较采用t检验、方差分析。Omentin-1与其他指标的相关性应用Pearson相关分析、多元回归分析。结果T2DM组及T2DM+IHD组患者血浆Omentin—I水平明显低于NGT组(P〈0.05)。血浆Omentin-1水平与BMI、HbA1c、HOMAH—IR、TG、LDL—C/HDL—C、IL-6呈负相关(P〈0.05),BMI、HOMA—IR、IL-6是影响血浆Omentin-1的独立危险因素(P=0.043,0.016,0.003)。结论血浆Omentin-1水平的改变与糖脂代谢及胰岛素抵抗有关,并可能参与了2型糖尿病合并缺血性心脏病的发生、发展。 Objective To investigate the changes and implications of plasma omentin-1 level in type 2 diabetes mellitus combined with isehemic heart disease. Methods A total of seventy male pa- tients with type 2 diabetes mellitus were divided into two groups according to whether complicated with is- chaemic heart disease, another thirty-nine healthy male control subjects were enrolled in this study. Plas- ma levels of omentin-1 were measured by enzyme-linked immunosorbent assay. Results Plasma omentin- 1 levels were significantly decreased in patients with type 2 diabetes mellitus[ (1164±10)pg/mll and in patients with type 2 diabetes mellitus combined with ischaemic heart disease [ (393±10)pg/ml ]compared with healthy control [ (5764 ± 10)pg/ml, P 〈 0. 05 or 〈0. 01 ]. Plasma omentin-1 level was negatively correlated with BMI, HbAle, HOMAH,IR, TG, LDL-C/HDL-C, IL-6 (P 〈 0. 05). Conclusions Chan- ges of plasma omentin-1 levels is related with glucose and lipid metabolism and insulin resistance, and it may involved in the pathogenesis of type 2 diabetes complicated with isehemic heart disease.
