摘要
目的探讨血浆尿酸水平与颈动脉内中膜厚度的关系。方法收集高尿酸血症患者78例,同时收集40例血浆尿酸水平正常且基线资料与高尿酸血症患者相匹配的研究对象作为对照组。采用超声检测双侧颈总动脉内中膜厚度。高尿酸血症患者予碳酸氢钠(1.0g,tid)和别嘌呤醇(100mg,bid)口服治疗,同时给予控制血压/血糖、调节血脂和抗血小板等心脑血管疾病一级预防,随访6个月按上述方法重复测量颈动脉内中膜厚度。结果 (1)与对照组相比,高尿酸组人群血浆尿酸水平明显升高,差别具有统计学意义(P<0.05);(2)与对照组相比,入院时,高尿酸组人群颈动脉内中膜厚度明显增大(P<0.05);治疗6个月后,血浆尿酸水平明显降低[(418±20)μmol/Lvs(587±46)μmol/L,P<0.05],且高尿酸组颈动脉内中膜厚度明显低于入院时[(0.84±0.06)mmvs(0.96±0.04)mm,P<0.05]。Pearson相关分析提示,血浆尿酸水平与颈动脉内中膜厚度呈正相关,相关系数r=0.864(P<0.05)。结论血浆尿酸水平与颈动脉内中膜厚度呈正相关,检测血尿酸水平可以反映颈动脉硬化程度。
Objective To investigate the correlation of serum level of uric acid （UA） with carotid intim-media thicknes （CIMT）. Methods Seventy eight cases of patients with higher serum level of UA and 40 cases with normal serum level c UA （control group） whose basic information was matched with patients of the higher serum level of UA were enrolled. Bi lateral CMIT of common carotid artery were detected by ultrasound. Patients with higher serum level of UA were prescribe, with sodium bicarbonate （1.0 g, tid） and allopurinol （100 rag, bid） orally, and were also given with first prevention of car diaccerebra vascular diseases by controlling of blood pressure and serum level of glucose, regulation of lipid and anti platelet. 6 months later, CIMT was detected as above again. Results （1） Compared to the control group, serum level of UI was significantly higher in patients with higher serum level of UA （P 〈 0.05）, （2） compared to the control group, CIMT at ini tial admission was significantly higher in higher serum level of UA group （P 〈 0.05）. With 6 months of therapy, serum lew of UA was significantly reduced compared to that at the initial admission [（418＋20） μmol/L vs （587_＋46）μmol/LP 〈 0.05 and CIMT was also decreased when compared to that at the initial admission [（0.84±0.06） mm vs （0.96±0.04） mm,P 0.05]. Serum level of UA was positive correlation with CIMT as indicated by Pearson correlation analysis （r = 0.864, P 0.05）. Conclusion Serum level of UA was positive correlation with CIMT and the evaluation of serum level of UA could n flect the sclerosis of carotid artery.
