摘要
目的评价微波原位灭活骨盆肿瘤的效果及髋关节功能。方法边缘外分离骨盆肿痛,应用微波针加热原位灭活肿瘤,刮除病灶,再用骨水泥塑形,观察肿瘤复发、转移情况,评价肢体关节功能。结果随访我院2005年1月~2011年6月,共24例微波原位灭活术治疗的骨盆肿瘤患者,平均随访2年5个月,其中成骨肉瘤3例、尤文氏肉瘤2例、软骨肉瘤7例、骨巨细胞瘤4例、恶性纤维组织细胞瘤3例,转移癌5例。结果 6例局部复发,10例出现远处转移,其中7例死亡,未出现复发转移患者有11例,髋关节功能优者11例,尚可者3例。结论外科手术是治疗骨盆肿瘤的主要手段,但由于解剖复杂、重建困难等原因,而出现术后局部复发率高、关节功能差等问题。应用微波原位灭活技术治疗骨盆肿瘤,不但能较好的灭活肿瘤、降低了局部复发率,而且保全了骨盆环的完整性,保留了较好的髋关节功能。
Objective To assess the effectiveness and joint function of pelvic tumors treated by thermotherapy with microwave antenna induced destroy in situ. Methods The pelvic tumor was separated with safe margin then inserted microwave antenna, which induced hyperthermia. The necrotic tissue was removed and the cavity was filled in the mixture of bone cement and allograft bone. All cases were observed the recurrence of tumor, metastasis and the joint function. Results 24 patients with the bone tumors occurred at pelvic were surgically treated form Jan 2005 to Jun 2011 with 2 years 5 months average follow - up times. Of the 24 patients, 3 patients were osteosarcoma; 2 patients were Ewing's sarcoma; 7 patients were chondrosarcoma; 4 patients were giant cell tumor; 3 patients were malignant fibrous histiocytoma; 5 patients were metastatic carcinoma. There were 6 local recurrence, 10 metastasis, in whom 7 dead. And 11 cases have neither recurrence nor metastasis. The hip joint functions of 14 patients were assessed. 11 patients were excellent, 3 patients were fair. Conclusion Surgical excision is the most important method in the treatment of pelvic tumor. But, there are many difficult problems in treatment for complex anatomy and reconstruction. The thermotherapy by inserted microwave antenna induced destroy in situ to treat the pelvic tumors, which can destroy the tumor issue, reducing the risk of operation and keeping the integrity of pelvic ring, is an effective and practical surgical method.
